This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
Duties
The Registered Nurse (Long Term Care) provides a full range of nursing care to patients/residents with complex care needs utilizing the nursing process with a resident/patient centered care model within patient care settings.
The RN-LTC can work on a variety of our 7 neighborhoods which include specialties like Hospice & Palliative Care, Dementia Care, Short-Stay and Long-Stay residents. Long Term Care Units Include: 5A/B, 5C, CLC-A, CLC-B, Tranquility House and Serenity House.
Duties and responsibilities include, but are not limited to, the following:
- Performs initial assessments and re-assessments of conditions and responses to treatment as well as any other clinical findings.
- Assess residents/patients learning needs; develops and implements plans to meet patients needs; educates residents/patients and families in self-care and treatment measures.
- Promotes continuity of care through developing an individualized interdisciplinary plan of care in collaboration with the resident/patient, family and health care team.
- Administers prescribed therapies using appropriate judgment.
- Accurately documents in patient's medical records findings, assessments, and care provided.
- Administers medications and treatments in accordance with established procedures and policies.
- Educates colleagues and/or students and serves as a preceptor or mentor.
- Initiates and participates in quality improvement activities.
- Serves as a role model when assigned as team leader or charge nurse under the supervision of the Nurse Manager or designee.
- Adheres to established policies, standards of care, and standards of practice;
- All other duties as assigned.
