Job Description:

The MDS Coordinator is responsible for coordinating and completing the Minimum Data Set (MDS) assessment for all residents in a skilled nursing facility.

The MDS Coordinator will work closely with the interdisciplinary team to ensure accurate and timely completion of the MDS assessment and care planning process.

The MDS Coordinator will also be responsible for ensuring compliance with all state and federal regulations related to the MDS assessment process.

Key Responsibilities:
Coordinate and complete the MDS assessment for all residents in a skilled nursing facility
Work closely with the interdisciplinary team to ensure accurate and timely completion of the MDS assessment and care planning process
Ensure compliance with all state and federal regulations related to the MDS assessment process
Provide education and training to staff on the MDS assessment process
Participate in quality improvement initiatives related to the MDS assessment process

Qualifications:
RN or LVN
Minimum Data Set experience
Skilled Nursing Facility experience
Strong attention to detail
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
Strong organizational and time management skills

Pay wage:
$2500 Sign-On Bonus
$50 - $55 an hour depending upon experience.
