The Domiciliary Mental Health (MH) Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
The RN assesses, plans, implements, and evaluates care based on age specific components.The incumbent assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
The RN follows procedures per established policies and guidelines and influences care outcomes by collaborating with members of the interdisciplinary team.
The incumbent executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.Duties include but are not limited to:
Assists in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation
Applies patient, family, and caregiver knowledge, values, and beliefs into care delivery without judgement or discrimination
Provides patient-centered and recovery-oriented care and maintains self-awareness while caring for patients living with substance use disorder and/or MH conditions
Facilitates nurse-led psychoeducational groups
Uses therapeutic communication techniques, practices de-escalation techniques, and utilizes recovery-oriented language
Provides oversight of the patient safe medication management program
Leads the provision of nursing care to patients with complex care needs utilizing the nursing process in alignment with the ANA Standards of Nursing Practice
Applies evidence-based practice/research resulting in improved outcomes in the immediate practice setting
Improves Veteran/patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting
Serves as an advocate for patients, families, and caregivers, improving outcomes for the immediate practice setting.
Pay:
Competitive salary, regular salary increases, potential for performance awards
