  • Compensation: Compensation is determined based on experience.
  • Benefits: Medical:
    -80% of monthly medical premium costs covered for single plans, 75% covered for individual plans
    -CHCB covers 80% of the Deductible costs for a single plan and 75% of the deductible costs for a family plan through a Benefit Debit Card.

    Dental:
    -Preventive dental services covered 100%, basic dental services covered 80% and major dental services covered at 50%.

    Vision:
    Yearly eye exam $10 deductible, lenses covered in full, $150 frame allowance. Frames or lenses subject to $25
    deductible. Contact lenses reimbursed $150 if elective, if medically necessary, covered in full.

    QUALIFICATIONS:
    -New graduates are welcome to apply however, candidates must have prior experience with women's health and/or family planning. This experience could be from the RN level
    -Licensed in NY state as a Nurse Practitioner
    -Understanding the role to be focused on family planning with women and men of reproductive age and up
