- 2+ years of outpatient clinical experience post-residency
- Current, unrestricted State license or eligibility for licensure
- Board certification/eligibility in Family Medicine or Internal Medicine
- Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) certification
- Active or eligible DEA registration
- Active Medicare/Medicaid enrollment or eligibility to enroll
- Effective oral and written communication skills
- Familiarity with Electronic Medical Record (EMR) system; proficiency in Athena preferred
