The **Registered Nurse in Home Health** provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with laws, regulations, and agency policies. They coordinate the total plan of care with other health care professionals involved and help maintain continuity of care.

1. Clinical Competence

  • Collaborates with the physician and healthcare professionals on the plan of care
  • Supervises care provided by home health aides and assigns tasks

2. Documentation and Care Delivery

  • Provides high-quality clinical services within infection control standards
  • Completes assessments accurately and timely
  • Documents patient visits thoroughly and per policy

3. Quality

  • Makes nursing evaluations, ensures patient eligibility, and communicates effectively
  • Coordinates care to ensure quality service excellence

4. Professionalism

  • Approaches work with a positive attitude and willingness to learn
  • Participates actively and punctually in all meetings

5. Teamwork

  • Completes tasks timely and professionally
  • Assists in orienting new personnel and participates in improvement activities
  • Participates in on-call and weekend rotations as needed

License Requirements:

  • Current RN licensure in the state of practice
  • Current CPR certification
  • Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation

State Requirements (LA):

  • Minimum one year of clinical experience as a Registered Nurse
  • No other state-specific requirements
