JOB DESCRIPTION: REGISTERED NURSE

Status: Exempt

Reports To: Lead Registered Nurse

HOPE Services Hawaii is seeking a full time registered nurse (RN) to become part of a dynamic, interdisciplinary team with direct report to the supervising medical provider. The RN works with houseless individuals in our community, offering both urgent and continuous medical and behavioral health care. HOPE Services Hawaii is committed to delivering high-quality nursing and care management in various non-traditional settings including but not exclusive to outdoor community areas, homeless shelters, and resource centers. The role requires a professional who is flexible, compassionate, self-motivated, and dedicated to our mission of advocacy and empowerment.

The Registered Nurse must be prepared to perform the following duties:

  • Observe strict compliance with HIPAA privacy and security rules for protected health information.
  • Accompany Street Medicine Team on once-weekly night missions as needed to assess, treat, and educate consumers in need of medical assistance according to licensure regulations and scope of practice; arrange follow up care as appropriate.
  • Provide health and wellness encounters at HOPE Shelters as scheduled to assess, treat, and educate guests in need of medical assistance according to licensure regulations and scope of practice; arrange follow up care as appropriate.
  • Document all services provided per agency standards and participate in tracking and reporting activities to support effective program management.
  • Provide in-service trainings to increase staff knowledge of health care issues impacting the target population and to promote effective communication and care.
  • Engage individuals with substance use disorder and mental illness with a strong commitment to harm reduction principles and trauma-informed care.
  • Track appointments, referrals, and outpatient healthcare activities and follow up with individuals to promote optimal health self-management.
  • Attend mandatory organizational meetings for the purposes of staff, program, and organizational development.
  • Performs other duties as deemed necessary by supervisor.

Specific duties include but are not exclusive to:

  • Physical, functional, psychosocial and cognitive assessment of individuals.
  • Provide support to medical providers to assist with administering medications.
  • Review hospital and agency referrals to HOPE Services for determination of placement in a medical program or non-medical shelter bed.
  • Establish achievable goals and interventions to be integrated into the individual's Master Individualized Recovery plan.
  • Promote and empower meaningful choices and service access options, as well as allow the service consumer to influence the type, duration, frequency and intensity of supports.
  • Facilitate collaboration between the individual and the multidisciplinary team to increase patient capacity for independently self-managing medical and behavioral health conditions.
  • Evaluate an individual's response to care based on continuous assessment.
  • Provide skilled nursing care, preventative rehabilitative procedures, and prescribed treatments to patients within home and community situations.
  • Identify patient's need for health education or coaching and provide as appropriate or link with community resources.
  • Identify any underlying substance abuse disorders and mental illness and encourage access to treatment resources including counseling, therapy and recovery programs.
  • Support the right to refuse or revoke services and/or seek restitution or grievance when an individual feels services are inappropriate or inadequate.
  • Support laboratory monitoring of medication efficacy by establishing prompts according to community standards.
  • Provide support for ambulatory detox as assigned.
  • Provide educational support for individual self-management of chronic health conditions, prevention of communicable diseases and general health maintenance.
  • Participate in the development of healthcare protocols with the interdisciplinary team.

The Registered Nurse must have knowledge and experience or be willing to undergo training in the areas of:

  • Environmental and personal healthcare safety regulations
  • Health care, mental health care, substance use, and addiction care and services;
  • Legal requirements and risks to perform this type of work, including the relevant legislation that impacts decision-making in particular situations;
  • Rental housing and requirements of tenants and landlords by law;
  • Homelessness, especially chronic homelessness;
  • Diverse cultures and impacts on cultural identity;
  • Economic poverty;
  • Income support services;
  • Child welfare;
  • Harm reduction;
  • Trauma and abuse;
  • Domestic and intimate partner violence (victim and perpetrators);
  • Life changes and ageing;
  • Fetal alcohol spectrum disorders;
  • Brain injuries;
  • Privacy and confidentiality;
  • Corrections and criminal justice.

In the course of performing the duties of the RN it is not uncommon to witness, engage in, or be confronted with the following: Violence and threats of violence; profane, racist, or sexist language; bodily fluids; conflict; interactions with First Responders; alcohol; street drugs; tobacco smoke; death of service participants or her/his associates; nudity of service participants or her/his associates; friends/family dynamics with service participants; people involved with sex work; people involved in the drug trade; persons used against their consent, will or knowledge; people in conflict with the law; and/or other situations that may be unsettling. Measures are taken to train staff to appropriately deal with these situations, but those in the position should reasonably expect these types of things to occur and understand appropriate direction and support will be provided by HOPE Services Hawaii leadership.

Minimum Qualifications:

  • Current, unencumbered state of Hawaii RN license
  • Preferred two years of nursing experience
  • Interest and experience in caring for underserved populations.
  • Past experience working with vulnerable populations and a desire to bring one's whole self to the service of our community.
  • An ability to collaborate effectively in a multicultural community.
Candidates must display and/or demonstrate physical ability to:
  • Engage with a computer keyboard and look at a computer monitor for several hours per day, and/or, exercise with comparable technologies like tablets;
  • Climb and/or walk up and down stairs and/or walkways.
  • Sit for periods of up to 30 minutes at a time.
  • Walk and/or standing for up to 30 minutes at a time.
  • Lift, carry, pull and/or push items up to 30 pounds at a time and for short distances. Such motions may include reasonable bending, stooping, reaching and twisting.
  • An ability to remain calm and productive in times of stress.

Starting Pay: $70,000/yr-$90,000/yr
