Provides Hospital At Home/Mobile Integrated Healthcare services. Provides primary care and/or advanced life support,including medical evaluation, treatment and stabilization of thecritically ill and injured with the goal of reducing morbidity.Supports existing health services; provides integrated healthservices in partnership with other health professionals; extendsaccess to health services delivery in underserved and generalpopulations, including primary care, public health, diseasemanagement, prevention and wellness, mental health, and dentalhealth; and performs other duties as required. The position islocated in the South Shore Hospital Emergency Medical Service (EMS)department.

JobDescription

ESSENTIALFUNCTIONS

1.Performs all Primary Job Responsibilities listed forParamedic.

2. Examines, screens, treats andcoordinates health services for patients.

3.Conducts post-hospital release follow-up care including, but notlimited to, monitoring medication, dressing changes, and checkingvital signs.

4. Observes, records, andreports to physician, patient's conditions and reactions to drugs,treatments, and significant incidents.

5.Conducts patient education, including diabetesprevention/treatment, hypertension, Congestive Heart Failure (CHF),Chronic Obstructive Pulmonary Disease (COPD), falls assessment,injury evaluation, geriatric frailty visits, andnutrition.

6. Administerspatient care consistent with department protocols and physicianorders.

7. Coordinates appointments andfollow-up with physicians and hospitals.

8.Develops and completes appropriate reports and templates for theCommunity Paramedic Program.

9. Attendsmeetings as requested and available.

10.Cleans and maintains (minor maintenance) vehicles.
a.Maintains records of vehicles, supplies, training and dailywork.

11. Completes Hospitalat Home Admission Process while using In patient platform of EPIC.Primary in home care giver partnered with Virtualnursing.

12. Collaborates withHospital at Home Hospitalist and VirtualNurses.

13. I Stat/Ultrasoundguided IV Competencies.

14.Participates in team huddles related to hospital atHome

15. Participates inHospital at Home related training and staffmeetings

16. Remote Patientmonitoring set up and troubleshooting.

17. ProvidesHospital At Home Education to patients and their families in thehospital at homesetting.

JOBREQUIREMENTS

Minimum WorkExperience
At least two years' EMS experiencepreferred.

Required Licenses/Registrations

ACLS - EMS
BLS - EMS
VDL - Valid DriversLicense

Verification of an acceptable motorvehicle driving record, at time of hire, with annual verificationrequired thereafter.

EMT- P - EmergencyMedical Technician-Paramedic

Requiredadditional Knowledge, and Abilities
Moderateword processing, database, presentation and spreadsheet skillsrequired.
Basic email and Internet skills required.Ability to learn specific job related software applications.
Must maintain a high degree of academic and practicalknowledge in emergency paramedicine, and must attend sufficientcontinuing education classes, courses and seminars both on and offduty to maintain annual paramedic certification, as required by theState ofMassachusetts

LANGUAGESKILLS
Ability to effectively speak and presentinformation before groups of customers, employees, and/or boards ofdirectors of the organization. Ability to read and interpret avariety of documents and instructions such as safety rules,operating and maintenance instructions, and procedure manualsfurnished in written, oral, diagram, or schedule form. Ability towrite routine reports and correspondence. Ability to read, analyze,and interpret common scientific and technical journals. Ability torespond to common inquiries or complaints from customers,regulatory agencies, or members of the business community. Abilityto read and interpret documents such as protocols, physicianorders, safety rules, operating and maintenance instructionsprocedure manuals;

REASONING ABILITY
Ability to apply common sense understanding to carry outinstructions furnished in written, oral, or diagram form. Abilityto solve practical problems and deal with a variety of concretevariables in situations where only limited standardization exists.Ability to interpret a variety of instructions furnished inwritten, oral, diagram, or schedule form. Ability to defineproblems, collect data, establish facts, and draws validconclusions. Ability to deal with several abstract and concretevariables in problem solving. Ability to prioritize tasks anddeadlines and apply critical thinking skills to address conflictingpriorities. Ability to interpret electrocardiogram (EKG) charts.Ability to practice sound decision-making skills
