Regions Hospital is seeking an experienced RN to join our Care Management department in an RN Medicaid Care Manager position. This RN CM will focus on continuity of care to monitor and manage high needs of high utilizing populations that have been identified for potential U-Turn opportunities. The RN CM will focus on our Medicaid population with a smaller case load and be the point person for updating Continuity Care Plan and contact external agencies and co-managing patients in the population in the hospital setting.

Work Schedule:

Monday- Friday 8 hour days no weekends

Required Qualifications:

  • Associate's degree of nursing with RN licensure from an accredited college
  • 5 years' experience working as an RN Care Manager or 3 years working as RN Care Manager within HealthPartners organizations.
  • Current State of Minnesota Nursing Board RN license

Preferred Qualifications:

  • Bachelors or Master's degree of Nursing
  • Certified Case Manager through Commission for Case Manager Certification (CCMC) or eligibility to sit for CCM exam within 2 years of hire.
