Clinical Pharmacy Specialist

The Harm Reduction Coordinator (HRC) Clinical Pharmacist Practitioner (CPP) is responsible for providing leadership, direction, organization, planning, implementation, and evaluation of harm reduction efforts at the Southeast LA Veterans Health Care System (SLVHCS). This includes but is not limited to overdose education and naloxone distribution (OEND), syringe services program (SSP), and fentanyl test strip distribution initiatives.

Major duties for the position include (but are not limited to):
- Developing, implementing, expanding, and evaluating outcomes of harm reduction initiatives: overdose education and naloxone distribution (OEND), syringe services program (SSP) and Fentanyl test strip distribution system;
- Identifying vendors and sources to obtain harm reduction supplies and harm reduction opportunities via clinical dashboards
- Partnering, collaborating and consulting with key facility services and programs, such as Housing and Urban Development-VA Supportive Housing (HUD-VASH), Addiction Recovery Treatment Services, local/national community harm reduction organizations for program improvement/partnership; Office of General Counsel (OGC) District Counsel, National VA office such as VA Hepatitis and Related Conditions (HRRC);
- Participating in relevant local, regional and national workgroups and advocating for policy and program changes to enhance harm reduction services for veterans;
- Providing direct patient care equivalent to 20% of time through direct outreach to veterans, staff referrals and veteran self-referrals;
- Providing Comprehensive Medical Management (CMM) focused on harm reduction;
- Screening, testing and treating drug-related infections, such as human immunodeficiency virus (HIV), hepatitis C virus (HCV), and sexually transmitted infections (STIs);
- Identifying patients who may benefit from CMM focused on harm reduction through population health management reports and team huddles;
- Conducting electronic consult (e-consult) medial record reviews and communicating recommendations to the care team;
- Educating veterans and health care system staff about harm reduction, OEND, and SSP initiatives;
- Leading and participating in pharmacy service quality improvement, process improvement, and research projects;
- Precepting pharmacy resident and/or intern quality improvement projects, presentations, and publications.
