Job Summary and Qualifications

The Manager Case Management is responsible for providing management support to all case management and access team members while ensuring accountability for performance and achievement of predetermined outcome targets. We are an amazing team that works hard to support each other and are seeking a phenomenal addition like you who feels patient care is as meaningful as we do.

What qualifications you will need:
  • Bachelor's Degree in Social Work or Health-related field OR BSN with active/current GA Licensure required; Master's Degree (MSW, MSN, MBA) preferred (Masters education requirement can be substituted with 8 years of work experience in case management)
  • 5+ years of acute care case management, utilization review, or social services experience required; 3+ years of leadership experience in a complex healthcare organization required
