Responsibilities

The Intensive Community Wrap Around Supports program is designed in partnership with the Department of Developmental Services and the Department of Mental Health to create intensive, in-home and community wrap-around services to support adults between ages 18 and 30 with Autism Spectrum Disorder and Serious Mental Illness, or other significant mental health challenges. The goal of this program is to maintain individual stability and effective functionality for people served in their homes and communities by providing clinical services and coaching supports to identify and respond to barriers of obtaining stability and support individual progress while building skills and capacity for everyone.

The PBS Qualified Clinician will serve as the primary clinician for the interdisciplinary ASD Community Treatment Team, which includes clinicians, nurses, treatment team coaches, and peer specialists. The clinician will complete all required clinical assessments within 30 days of admission to ensure timely and focused treatment planning, conduct functional behavioral assessments, and maintain all electronic records for each person served.

Other Responsibilities:

  • Provide training for the community treatment team to include autism spectrum disorder, evidenced based and promising practices to include PACT, Trauma Informed Care and Positive Behavioral Supports.
  • Support the ASD Community Treatment Team, individuals served and families in the development of individual care plans, safety plans and cope ahead plans.
  • Incorporate Positive Behavioral Supports (Universal Supports) into all elements of practice. Oversee the development of PBS Universal and Targeted interventions.
  • Participate in after hours on-call rotation to promote community access and stabilization for individuals served.


Qualifications
  • Master's degree in ociall Services field, required.
  • Must be licensed as a LMHC, LICSW LABA or BCBA.
  • Minimum of three years' clinical experience in the treatment of individuals with developmental disabilities, ASD, and significant mental health challenges, and knowledge of PBS, required.
  • Valid Drivers License, acceptable driving history and reliable transportation, required.

Compensation details: 70000 Yearly Salary
