Summary

The Licensed Practical Nurse/Licensed Vocational Nurse performs more complex nursing assignments within their setting. The LPN/LVN possess comprehensive practical nursing knowledge and skills in specialized procedures and practices. Individuals in these positions are to a considerable extent, expected to exercise independent technical judgments.



Duties

**This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until September 30th, 2024. The initial cut-off date for referral of eligible applications will be January 31st, 2024, with subsequent cut-off dates every 2 weeks. Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available.**

The Patient Aligned Care Team (PACT) LPN/LVN works under the direction of a Registered Nurse (RN) and reports to the Primary Care Nurse Manager. The LPN/LVN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of patient driven holistic care plans for life. The LPN/LVN is a member of the extended care team.

Major duties and responsibilities include, but are not limited to:

  • Possesses the knowledge and skills to carry our more specialized patient care activities. This knowledge base includes the use of veteran care data to initiate active contributions to the veteran's overall health plan. Observations of veteran's present status are made, and interventions implemented to improve, restore, or maintain the optimal function of the veteran.
  • Contributes to the development of the plan of care for assigned veterans outlining program modalities and updates per policy and as changes occur. This plan of care is completed with general review and co-signature by the RN or MD, utilizing a broad knowledge base and expertise to develop and modify treatment plans to meet the veteran and the family's expectations.
  • Promotes and demonstrates nursing concepts in the Medical Center. Works in collaboration with the RN, and or Health Care Provider (HCP).
  • Prepared and administers prescribed medications (oral, topical, subcutaneous, intramuscular, and/or intravenous) and performs treatments according to established policies/procedures. The LPN/LVN observes for physical and/or emotional changes in patient's health condition.
  • Takes appropriate actions based on communications, observations, veteran care reports, and provider/staff discussion. Reports changes to the provider, and demonstrates the knowledge and ability to effectively intervene and recommend and implement strategies that assure optimal, sage, and effective care.
