Overview

The Community Health Needs Assessment (CHNA) Director will be responsible for leading the community health improvement planning process and supportive activities for the region by facilitating the development of community health needs assessments (CHNA) in compliance with section §501r of the IRS for hospitals across the region. This role will lead the coordination of gathering of community level data, analysis, and presentation to drive the assessment process in developing community health implementation strategy and outcomes. This role will have visibility and responsibility in service to local hospitals, health system senior leaders, board members, community steering committees and community health councils. The role partners with community health directors, facility leadership, key community stakeholders and other work teams to identify and prioritize community health needs in each market. The role also leads the development of the community health improvement strategy and yearly progress reports for all hospitals in the region. This role will lead coordination and ensure regulatory compliance working alongside internal teams for approved website postings and federal filings related to CHNA reporting.

  • Remote position with limited travel.

Major Responsibilities

  • Leads the CHNA survey process, development of external collaborations, and development of the CHNA reports including reporting to finance/hospital boards and developing and implementing each hospital's Community Health Implementation Plan (CHIP). Manages multiple state's CHNAs working with many internal departments and external entities, organizations, and community-based partners.
  • Leads broad scope public / community health data analytics in each hospital's primary and secondary service area which requires specialization in public health practice for the community health improvement processes. Leads the region's CHNA process including operations (daily management of processes, timelines, reporting, and outcomes measurement), implementation, and coordination of efforts across the region.
  • Works with executive leadership to interpret and present data in the context of each hospital's strategic community health goals. Works collaboratively across the region to provide consultation, analytical and project support to site-based community health teams for assessing community health needs and assets.
  • Leads the coordination of compiling CHNA data obtained for hospital market areas and assists with data presentation in CHNA reports to guide program planning and for public posting. Provides expertise to improve hospitals' participation in development of program metrics to support community health improvement plans.
  • Develops CHIPs which include setting strategy and developing measurable goals and targets for multi-year CHNA cycles. Responsible for execution and ensuring goals/targets are met which are critical to maintaining our non-profit tax status. Ensures appropriate program metrics are developed, analyzed, approved, and utilized by teams to measure progress and evaluate community health strategies.
  • Provides general research and analysis support to inform operations, funding, program ROI/success and policy-change; and maintains awareness of health inequities and issues impacting persons living in poverty compared to those impacting the broader community. Participates with hospital implementation teams and contribute to efforts to advance community health outcomes in each market.
  • Leads the analysis of implementation strategies and benchmarks against national standards and recommends adjustments accordingly.
  • Manages the relationship between assessment software and licensing (ex. Metopio) for teammates including access requirements, training, and onboarding. Manages relationships with other CHNA vendors or consultants as appropriate.
  • Provides guidance and support to local hospital leadership in establishing and leading community health councils incorporating representation from community leaders, non-profit agencies, and health departments. Responsible for coordinating community-surveys, understanding the data, and using data to determine community health needs to develop three-year strategic health improvement plans.
  • Supports leadership in obtaining board approvals for the CHNA reporting including preparing reports, presentations and documentation needed for multiple regional and local boards. Provides support to identify evidence-based programs to implement in each hospital market to meet the CHNA priority areas. Responsible for evaluation of all the regional plans and deliverables including health improvement outcomes measures. Responsible for developing an evaluation plan for health improvement programs.
  • Leads the posting of CHNA, CHIS and progress reports to meet the requirements of section §501r of the IRS. Ensures a high-level of accountability for the region's CHNAs across hospital sites. Responsible for meeting the federally mandated community health improvement process which includes community health data analysis to identify community-level health needs, prioritization of health needs in collaboration with hospitals and community members and development and execution of evidence-informed programs to improve needs identified from CHNA process.

Minimum Education and Experience Required

Level of Education: Bachelor's degree in Community Health, Public Health, Business Administration, Healthcare Administration, Public Administration, Healthcare Advocacy, or related field. Master's degree preferred.

Years of Experience: 10 years in healthcare, public health, health plan or non-profit work gaining the understanding of how to retrieve public health and community level data, analyze data and present in a meaningful format. Must understand the community health improvement process and program, metrics, and outcomes measurement.

Minimum Knowledge, Skills and Abilities (KSA)

  • Requires a very analytical skillset along with highly dynamic interpersonal skills to drive work across multiple states with both state, regional and county partners. Must have expertise in public health, health equity best practices, excellent written and oral communication skills, as well as the ability to lead at a regional level.
  • Demonstrated ability to independently research and analyze public health data and social issues and must be an experienced and highly skilled writer. Requires high proficiency in Word, Excel, and PowerPoint.
  • Demonstrated experience building effective, strong relationships with community organizations and leaders as well as coordinating relationships between the community and internal employees. Demonstrated ability to interact effectively with people at all levels of the organization.
  • Excellent oral and written communication skills, including public speaking.
  • Excellent writing, research, presentation, analytical, organizational, interpersonal and negotiating skills.
  • Knowledge of best practice approaches to community health improvement
  • In depth awareness of trends and issues related to health care industry, and health inequities in underserved and marginalized communities.
  • Skills in statistical and data analysis developing databases and dashboards.
  • Functional knowledge of clinical service delivery.

Physical Environment and Working Conditions

  • This position requires travel, therefore, will be exposed to weather and road conditions. Ability to travel throughout Advocate Health's region including various markets, assigned areas and hospital sites to provide training and education, and to collaborate with hospital and community partners.
