Duties include, but are not limited, to:

Incumbent provides the following:

direct outreach for the HCHV program in the Dayton VA and in the community; provides outreach services for the HCHV program in the community; training for other members of a multidisciplinary treatment team in areas related to anger management, life skills, and SUD with cooccurring disorders; necessary assessment functions, e.g.

interviewing, psychosocial histories, and to aid in the development of treatment plans to support the Homeless programs as a functional member of the team, to include consultation, interventions, or accompanying other HCHV personnel into the community; liaison services with other programs referring homeless Veterans to HCHV to ensure continuity of care and promote successful transition of Veterans from one program to another; consultative services to the HCHV team, as needed; appropriate clinical documentation for all contacts with or about Veterans; recovery-oriented treatment with the goal of establishing the Veteran independently in the community at the Veteran's highest level of functioning and quality of life.


Incumbent conducts intake and needs assessments of homeless Veterans and their significant others, develops comprehensive treatment plans with the Veteran that addresses factors that contributed to homelessness, including SUD and other co-occurring factors within a recovery model framework, works with the Veteran to realize treatment goals, and reassesses and revises goals as indicated by the Veteran's engagement in treatment, organizes and conducts individual and group case management and SUD treatment, provides homeless outreach services to sites both in the community and at the Dayton VA and CBOC's.


The incumbent will also be required to perform (25%) of their job duties in the community and will be required to utilize a GSA Vehicle while performing such duties.

The HCHV Outreach Social Worker will successfully complete all facility requirements to drive a GSA Vehicle and transport Veterans. This includes all pre-employment physicals.

In addition, the incumbent will successfully complete all facility required TMS trainings to drive a GSA Vehicle and transport Veterans.
