Duties

The Advanced Practice Registered Nurse (APRN) provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well-being; and executing medical regimens including administering medications and treatments prescribed by a licensed or otherwise legally authorized provider. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses).

Major duties may include, but are not limited to, the following:

  • Will be expected to Float between all Community Based Outpatient Clinic (CBOC) sites and the main Medical Center depending on coverage needs of the Organization.
  • Assesses, plans, implements and evaluates care based on age-specific components.
  • Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
  • Administers medications and procedures per established policies and guidelines.
  • Influences care outcomes by collaborating with members of the interdisciplinary team.
  • Collaborates with the PACT Teamlet (patient, Primary Care Provider, and Clinical Support Technician) and extended PACT Team (family/caregiver, internal and community-based services involved in providing care to the patient).
  • Teach health promotion and disease prevention to patients.
  • Diagnose and treat acute and chronic health care problems.
Work Schedule: 8am-4:30pm Mon-Fri (Excluding Federal Holidays) Saturday Rotation to cover clinic hours
