LPN Boise Extensivists Clinic Full-Time Days

The Extensivists Clinic is looking to hire a personable and efficient Medical Assistant or LPNto support our providers at our Boise Clinic located on the Regional Medical Center campus
***$5000 Sign On Bonus offered to new LPNs and $7500 offered to experienced LPNs to external candidates for this position***About the Extensivists Program:- The Saint Alphonsus Extensivists Program provides priority access and transitional care for patients with complex medical needs following acute inpatient hospitalization. This program is an extension of the hospitalist program and offers short term, high-intensity services for patients that have more needs, require more time, or need to be seen prior to what their primary care provider has available. Our team of specialists will work together with each patient to help ease the transition from the hospital to resumed care with a primary care provider.
- Extensivist program includes:
- An appointment at the Extensivist Clinic within 1-7 days of hospital discharge
- All coordination of care, including exams, tests, medications, and treatments
- Pharmacist consultation at each visit
- Treatment, education, and counseling to focus on preventing issues that cause frequent hospital readmissions and emergency room visits
- A summary of medical records and medications prescribed sent to the patient's primary care provider
As the LPN or MA for the Extensivists clinic you will be working with a very driven team of providers and pharmacist.Duties will include:
- Obtaining vital signs and patient histories; maintaining a complete and accurate patient medical record per Saint Alphonsus policy.
- Administer medication and perform phlebotomy in accordance with policy.
- Maintains a clean, safe, well-stocked nursing area, including patient examination rooms.
- Provides clinical telephone communication as defined by policy and per provider instructions.
- Review hospital discharge list to identify patients that qualify for the Extensivist program. Coordinate with provider and team to schedule new referrals to clinic.
- Performs clerical functions including prior authorizations, referrals, laboratory/diagnostic, referral tracking, and processing insurance forms.
An ideal candidate for this position is very personable and enjoys working with an older patient population, is detail oriented, a quick learner, has excellent organizational and prioritizing skills, strong communication skills, and has a passion for providing high-quality patient care. The ability to be adaptable and flexible, provide compassionate care while utilizing critical thinking skills will be essential in this position.
Work Schedule: This position will work 40 hours per week, Mondays - Fridays, 7:30am - 5:00pm.Learn more about the Extensivists Program: LPN REQUIREMENTSLicense and Certification Requirements:- Colleague must have an LPN license, as defined by their primary work state (Idaho or Oregon), issued prior to hire date. Those colleagues hired before January 1, 2020 must provide licensure or proof of application in process in their secondary state by August 28, 2020. Individuals hired in this role after January 1, 2020, must provide licensure or proof of application in their secondary state by August 28, 2020 or within six months of hire, whichever date is later.
- Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hire.
- Experience in an ambulatory care setting preferred.
- High School Diploma or equivalent required.
- MA certification required within 90 days of eligibility to take a national certification exam from one of the following Saint Alphonsus approved agencies:
- American Association of Medical Assistants (CMA)
- American Medical Technologists (RMA)
- National Healthcare Association (CCMA)
- National Center for Competency Testing (NCMA)
- American Registry of Medical Assistants (ARMA)
- Basic Life Support for Healthcare providers certification from either AHA, ARC or the Military Training Network is required at hire
- Previous clinical experience preferred.
- Electronic health record (EHR) experience a plus.
- Experience with process improvement or LEAN philosophy preferred.
