GLIN is seeking a Social Worker- Oncology to assist oncology patients, their families and/or caregivers with information on internal and external resources, financial, practical, and emotional concerns across the continuum. To coordinate all resources and services with the oncology multidisciplinary team overcoming health care barriers. Services are provided in accordance with professional standards and in consultation and/or collaboration with the multi-disciplinary health care team, as warranted, to assistant/encourage patients/families significant others to utilize medical care constructively to assure the achievement of an optimal level of functioning in oncology care.

Duties and Responsibilities (including but not limited to)

Support the psychosocial screening process for the patient, family and/or caregiver at pivotal medical visits across the continuum: prevention, diagnosis, treatment, survivorship, palliative care, end of life care and bereavement. (Commission on Cancer Standard 5.2)
Provide counseling and education regarding cancer diagnosis and treatment with the patient, family and/or caregiver, facilitating referrals to appropriate disciplines and community resources.
Assess and remove barriers to care across the continuum, coordinate with appropriate resources. (Commission on Cancer Standard 8.1)
Participate on the working group for the Cancer Focused Community Needs Assessment.
Work closely with the Manager of Survivorship & Supportive Services to create programs that improve patients' quality of life and outcomes from diagnosis through treatment and survivorship stages.
Assess gaps in services within the cancer program and community agencies.
Create screening tools that address psychosocial, social, cultural, and spiritual factors as needed.
Participate in the multidisciplinary center (MDC) visits with the patients and MDC team as needed to provide education and support regarding psychosocial issues.
Participate as the Psychosocial Services Coordinator for the Commission on Cancer meetings (Commission on Cancer Standard 2.1)
Facilitate referrals to the Survivorship provider and program as appropriate.
Collaborate with other team members on the Patient Advisory Committee
Assess, educate and/or refer the patient, family and/or caregiver on the following as needed:
Substance abuse/smoking cessation programs
Lodging during treatment periods
Special programs for caregivers, children and other members
Social Security Disability
Medicaid and Medicare Guidelines
Medicare Part D
COBRA Insurance
Family Medical Leave ACT (FMLA) and Paid Family Medical Leave Act (PFMLA)
Financial resources, agencies, and programs to assist with co-pay relief and other financial assistance, as needed
Transportation resources
Advance Directives/Health Care Proxy
Home care, Palliative care, Hospice, and end of life resources
Medications and Pharmaceutical Indigent programs
Hospital charitable organizations
Other duties as assigned

Qualifications or Education, Training, and Experience:
Work Experience
Licenses and Certifications
Masters of Social Work required
Valid and current LCSW or LMSW licensure
Oncology Social Worker Certification (OSW-C) within 3 years of hire date
Valid driver's license- this position works at multiple sites with some travel required.

Knowledge and Skills:

This position does NOT offer clinical supervision for a LMSW or hours for the LCSW licensure.

Timely and accurate documentation of day-to-day activities in designated technology platform.
Adaptable to new technologies and software.
Basic PC skills (MS Word/Outlook/PPT/Excel).
Knowledge of Federal and State regulations for Medicare and Medicaid and other national and state funded programs.
Knowledge of community resources access.

This position will be working onsite at multiple location in WNY and some traveling required
We offer an outstanding benefits package including health, dental, 401K, vacation, and PTO, as well as a great working environment
Pay range: $65,000 to $80,000
The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within job's compensation range, and will be determined by considering factors including, but not limited to market data, education, experience, qualifications, and expertise of the individual and internal equity considerations
