Registered Nurse-Patient Aligned Care Team(PACT)

Covers PACT teams as needed for scheduled and unscheduled leave.
Serves as a resource for clinical associates, ensuring quality of care delivery and overseeing patient throughput.
Participates in orientation and competency assessment of nursing staff.
Conducts nursing assessment following hospitalization and ED visit, and coordinates follow up care with internal and external care providers.
Identifies and manages high risk patients via face-to-face visits, virtual video connect, secure messaging and telephone calls.Coordinates care transitions with specialists and consultants including home care, palliative, or hospice care.
Partners with Veteran to support self-management, prevention and wellness, utilizing Health Coaching and Motivational Interviewing to assist with behavior change and guidance towards his or her goals.
Provides patient-centered, evidence-based, patient driven care in all patient interactions across the care continuum.Other duties as assigned.

Monday-Friday, Day Shift

May be available on an Ad-Hoc basis at the supervisor's discretion.
EDRP Authorized:
These positions are eligible for EDRP, a student loan payment reimbursement program.
You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of your start date.
Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

This is a Bargaining Unit position.
This position is in the Excepted Service.
Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held.
This position is covered by a special rate.
Pay:
