Major Duties Include but not limited to:
Communicates respectfully and works effectively with administrative staff, the Patient Aligned Care Team (PACT}, patients, and visitors.

Provides emotional support to patients, and families/significant others to assist them in adjusting to changes brought about by illness or change in health status.

Prepares/administers prescribed oral, topical, subcutaneous, inhaled, and injectable medications, and suppositories according to policy and procedure. Correctly documents medications given or omitted in Computerized Patient Record
System (CPRS) according to established policy and procedures. Documents effects of medications.
Recognizes emergent or acute situations, responds appropriately, and reports to the RN or provider promptly. Reports untoward patient incidents, including medication errors, to Charge Nurse/Nursing Supervisor, and initiates incident report. Responds appropriately to emergency situations including cardiopulmonary arrest, choking, and disaster or tire drills.

Observes, reports to the RN, and documents in the patient's medical record subjective and objective changes in the patient's physical or emotional condition, which have relevance in determining the health status of the patient.

Provides teaching in activities of daily living (AOL's) to patients and encourages them to participate in AOL's leading to increase independence.

Provides or reinforces patient teaching related to medications, disease process, procedures, and self-care activities, including more complex teaching, such as diabetic care, self-catheterization, etc.

Performs treatments and procedures utilizing established nursing care plan according to policy, including but not limited to:

range of motion exercises, application of heat and cold, wound care, changing complex sterile and clean dressings, weights, vital signs.

Collects specimens such as urine, sputum, and stool for laboratory analysis.
Sets up for and performs treatments that require auxiliary equipment, for example:
oxygen, spirometry, and nebulizer treatments.
Utilizes the Safe Patient Handling Program standards, practices, and recommendations.

Lifts and transfers, according to policy and procedure, seeking assistance when appropriate, to prevent injury to the patient and self.

Prepares patients for tests, examinations, and treatments technique.

These procedures include, but not limited to:
insertion of chest tubes, thoracentesis, paracentesis, lumbar puncture, and liver biopsies.
Applies assistive/protective devices such as, splints, binders, or prosthetic appliances.
Documents observations, therapeutic measures performed, and care provided following established policy and procedure. Collects subjective and objective data which contributes to the RN assessment and/or the patient reassessment.
Accurately uses appropriate senses in collecting patient data and providing care. Provides patient information to the RN for inclusion in the patient's treatment plan.

Participates in patient care planning and in discharge planning by obtaining and contributing information relevant to the patient's progress and needs.

Provides competent care which is safe for both self and others under adverse conditions, including patient care situations, exposure to communicable diseases, exposure to constant noise and working protracted or irregular hours.

Functions as a team leader under the supervision of the RN.

Serve as a responsible member of the nursing team and interact with patient, family members, professional and supportive personnel who provide patient care, and with members of other services.

Follows environmental policies and procedures related to the Erie VAMC GEMS program, recycling, and the safe use, handling and disposal of chemical products.

Follow all policies and procedures to ensure compliance with all applicable environmental regulations and requirements and be good stewards of the environment through pollution prevention, waste minimization and resource conservation.

Supports the Environment of Care Management Program by completing scheduled training on safety, health, environmental and fire protection requirements related to their assigned tasks; following safe work practices and procedures, including the use of appropriate equipment, and any required Personal Protective Equipment (PPE) item; recognizing and reporting unsafe and unhealthful conditions, initiating prompt corrective action within their authority, and promptly reporting work-related injuries or illnesses to supervisory personnel.
