Overview

JOB SUMMARY:

Provide social work services, complex discharge planning, and care coordination to pediatric hematology patients and their families. Identify needed community services and refer patients in need to the appropriate services. Collaborate with other members of the multi-disciplinary team to affect positive patient and family outcomes. Communicate with other hospital staff re: psycho-social issues that impact health condition/status.

EDUCATION/EXPERIENCE:

Master's degree in Social Work from a school accredited by the Council on Social Work Education (CSWE)

LICENSURE, CERTIFICATION, and/or REGISTRATION:

Clinical licensure (or provisional status) desirable Certification as an Accredited Case Manager preferred (ACM-SW) preferred

ESSENTIAL FUNCTIONS:

Pediatric Hematology Social Worker Job Duties

Shared Responsibility and General Hematology

  • Conducts needs assessments and psychosocial evaluations and facilitates resource connection
  • Works collaboratively with interdisciplinary team of providers to ensure patients' needs are met
  • Assess for SDOH barriers and provide intervention, including transportation, food insecurity, etc.
  • Engages with patients who have identified mental health needs and concerns to provide brief intervention and connect with providers within their community
  • Engages with school and daycare partners to ensure proper documentation of patients diagnosis and provision of appropriate accommodations (504/IEP)
  • Maintains up to date knowledge of resources and supports available to patients and their families
  • Assists with completion of documentation for supports such as FMLA or SSI
  • Maintains general knowledge of insurance processes and provides education to families when necessary
  • Participates in available continuing education conferences and advocacy opportunities
  • Maintains knowledge of wish granting organizations' eligibility criteria and completes patient referrals as appropriate
  • Engages with other social workers within the organization to coordinate care

Hemophilia Treatment Center/Bleeding Disorders

  • Engages with multidisciplinary care team as part of shared comprehensive annual patient visits
  • Conducts annual psychosocial assessment for all HTC patients
  • Collaborates with community organizations such as BDFNC to connect patients with resources and programs
  • Transition
    • Identifies patients who qualify for transition program
    • Engages in education with patient and family about transition objectives and goals, health privacy changes at 18, and adult insurance
    • Administer, assess, document, and evaluate transition readiness assessments
    • Collaborates with multispecialty team, patient, and family to provide needed intervention and education to prepare patent to transition to adult care
    • Maintains accurate data collection and records in internal (Redcap) and external (ATHN) databases
  • Participates in monthly multidisciplinary team meetings to collaborate on patient care and program development

Sickle Cell

  • Works collaboratively with Sickle Cell Disease team to identify patients who need psychosocial intervention
  • Collaborates with community organizations such as Piedmont Sickle Cell Agency and Saddle Up for Sickle Cell to connect patients with resources and programs
  • Engages in programming of events for sickle cell patients and community, such fall picnic to recognize sickle cell awareness month
  • Engages in advocacy for resources and further supports for our patient population
  • Engages with multidisciplinary care team in the development and implementation of a transition program

Demonstrates competence related to age and developmentally appropriate care Establishes positive work relationships and works to reduce work place conflict Demonstrates ability to work in fast paced environment with multiple interruptions Embraces change in rapidly changing health care environment Social work experience in a healthcare setting desirable
