The RN Case Manager builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Evaluates patient's eligibility on admission and on-going certification of patient by analyzing health history and physical assessment.

  • Assists with construction of a patient centered plan of care and makes necessary revisions as patient status and needs change.

Plan of care includes:
A.
Understanding of hospice, illness, and prognosis B.
Goals of care/advance care planning/proxy decision-making C.
Detailed symptom history D.
Psychosocial and coping history including loss history E.
Spiritual history F.
Functional assessment G.
Chart Care plan monitoring for accuracy and appropriate response when errors are discovered H.
Pharmacologic history including substance dependency or abuse 3.
Monitors preventive and rehabilitative nursing procedures.

  • Orders medications and oversees patient symptoms and pain in GIP setting.
Communication 1.
Reviews field nurse clinical notes regarding patient's condition and care given.
Obtain orders as needed.
Records pain/symptom management changes/outcomes as appropriate.

  • Communicates with the physician regarding discharges.
  • Communicates with community health related persons to coordinate the care plan.
  • Works in concert with the interdisciplinary group by generating IDT agenda and checking for IDT signatures.
Attends IDT group meetings.

  • Assists the team members in providing continuity of care and monitoring effectiveness.
Additional Duties 1.
Participates in on-call duties as defined by the on-call policy.

  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Actively participates in quality assessment performance improvement teams and activities.
  • Provides documentation and record keeping in accordance with professional standards and organizational policies.
  • Reviews competencies for registered nurses if necessary.
POSITION QUALIFICATIONS 1.
Bachelor's degree; graduate of an accredited school of nursing.

One (1) to two (2) years of recent acute care experience in an institutional setting, and One (1) to two (2) years of recent experience in home care.

One year of hospice care experience preferred.

  • Current licensure in State and CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
  • Management experience not required.
May be responsible for supervising hospice aides.

  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
