About the Opportunity

Join Pair Team in revolutionizing the delivery of primary care to vulnerable populations. We are seeking a dedicated Lead Care Manager to join our team and make a difference in how people access healthcare services.

The ideal candidate will lead our interdisciplinary care approach, focusing on engaging individuals facing mental health issues, homelessness, or complex medical needs.

This position offers a remote-forward work environment with occasional on-site requirements in Merced, California.

Responsibilities
  • Manage a caseload of individuals, using evidence-based strategies to support their health goals
  • Utilize relationship-building techniques to engage members and guide them through the healthcare system
  • Conduct regular outreach to ensure timely follow-up and support for individuals
  • Collaborate with a multidisciplinary team to create and implement personalized care plans
  • Identify and address barriers to care, ensuring continuity in individuals' participation in the program
Requirements
  • 3+ years of work experience, preferably in Case Management
  • Located in or near Merced, California
  • Experience in case management and bilingual in English and Spanish
  • Strong empathy and cultural fluency
  • Ability to work collaboratively and adapt to different situations
  • Organizational skills and a problem-solving mindset
  • Comfortable with technology and basic software applications
  • Valid driver's license and auto liability insurance

  • Experience working with individuals with complex needs or mental health challenges
  • Knowledge of motivational interviewing and medical terminology
  • Detail-oriented and self-motivated
  • Comfortable in a dynamic work environment
