The ideal candidate will lead our interdisciplinary care approach, focusing on engaging individuals facing mental health issues, homelessness, or complex medical needs.
This position offers a remote-forward work environment with occasional on-site requirements in Merced, California.
Responsibilities
- Manage a caseload of individuals, using evidence-based strategies to support their health goals
- Utilize relationship-building techniques to engage members and guide them through the healthcare system
- Conduct regular outreach to ensure timely follow-up and support for individuals
- Collaborate with a multidisciplinary team to create and implement personalized care plans
- Identify and address barriers to care, ensuring continuity in individuals' participation in the program
- 3+ years of work experience, preferably in Case Management
- Located in or near Merced, California
- Experience in case management and bilingual in English and Spanish
- Strong empathy and cultural fluency
- Ability to work collaboratively and adapt to different situations
- Organizational skills and a problem-solving mindset
- Comfortable with technology and basic software applications
- Valid driver's license and auto liability insurance
- Experience working with individuals with complex needs or mental health challenges
- Knowledge of motivational interviewing and medical terminology
- Detail-oriented and self-motivated
- Comfortable in a dynamic work environment
