Main Purpose of Job

The main purpose of this position is to coordinate care by helping individuals transition from substance abuse residential treatment to community-based aftercare, recovery-based housing and support. This position also initiates and supports reentry planning for individuals incarcerated within the local detention center. This position� will connect individuals to recovery services in their community by providing patient screening, assessment, care planning, referral and placement.\r

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate. \r
Experience: One year of administrative staff or professional work in a health care setting. \r
Notes: \r
1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience. \r
2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education. \r

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess the following:\r

Experience working with incarcerated individuals who seek transitioning guidance from residential treatment to community based housing.\r
Experience coordinating care for individuals with behavioral health conditions.\r
Experience connecting consumers to community based providers and other recovery support services.\r
Experience/knowledge, skill and ability to develop and maintain relationships with consumers and community providers.\r
