Overview:

With approximately 40,000 admissions per year and 6,000 average daily population, the NYC jail system is among the largest in the nation. The Division of Correctional Health Services (CHS) of the New York City Health and Hospitals (NYC H+H) is responsible for the delivery of medical care, mental health care and discharge planning in the jail system. CHS is a national leader in the delivery of healthcare to the criminal-justice involved population and strives toward the highest standards of care, with the goal of meeting or exceeding what is available in the community.


From the comprehensive history and physical at the point of admission, to robust screening and treatment efforts around HIV, hepatitis C, STDs and TB, to the management of chronic disease, CHS views incarceration as a crucial point in time to provide essential care to an underserved population often not engaged in care in the community. In this way, CHS medical seeks to both provide the highest level of care to our patients and to improve the population health of the communities to which they return.


CHS is committed to continuously improving the delivery of care in the security setting, using the system-wide Electronic Health Record as a tool to promote patient safety and population health. CHS has contributed to the literature on medical care in jail and public health implications of incarceration. CHS seeks to build on a reputation as a national leader in supporting patient care by including integrated networks and harnessing the expertise of the City’s public hospital system in caring for our patients. CHS Medicine seeks to promote an academic practice environment with ample opportunities for continuous education, clear pathways for career growth and engagement of front line staff in the ongoing process of improving clinical care.


Purpose of Position:

Under general supervision with considerable latitude for the exercise of independent initiative and judgment, works as a Site Medical Director overseeing the provision of medical care within one of the New York City jail facilities and supervises Telehealth Services provided across facilities.


Examples of Typical Tasks:

  1. Manages, supervises, coordinates and evaluates operations of jail-based health care. May also supervise and coordinate professional, clinical, special services, support services, or other related services.
  2. Responsible for overseeing clinical operations of the facility including processing of daily work such as laboratory and radiology review, medication renewal and referral reviews. Plans, organizes and supervises comprehensive medical services and coordinates the work of professional staff.
  3. Regularly meets with telehealth physicians to review performance, call data and to provide updates on workflows.
  4. Regularly conducts Quality Assurance reviews and compiles data about telehealth utilization and workflows. Leads Telehealth Quality Improvement initiatives.
  5. Chairs telehealth staff meetings with staff on a regular basis.
  6. Consults with other CHS departments (e.g. Nursing, Operations, Monitoring & Evaluation) on telehealth protocols and collaboration.
  7. Serves as clinical educator for front line clinical staff on evidence-based medical practice, workflow, application of policy, and quality improvement efforts.
  8. Provides direct clinical care in order to understand clinical workflows, implications of policy changes and application of community standards to the security setting, as well as in exigent circumstances.
  9. Serves as liaison between CHS and NYC Department of Correction staff as relates to clinical issues.
  10. Retains current knowledge of latest evidence based community standards of practice.
  11. Coordinates care with outside hospitals and specialty services.
  12. Participates in Quality Improvement initiatives and recommends changes in policy and workflow for more efficient, evidence-based practice.
  13. Develops operating plans, programs and procedures to implement system-wide goals and objectives.
  14. Develops facility policies and procedures, and translates administrative policies, goals and objectives into working programs. Assists in interpretation and implementation of CHS policies and procedures.
  15. Provides recommendations and direction concerning proposed projects and/or related programs department heads.
  16. Assists in the development of budgets and allocation of funds based on studies of costs, reviews of departments/divisions, familiarity with operating procedures and discussions with department heads.
  17. Resolves such problems relating to staffing, utilization of hospital facilities, equipment and supplies.
  18. Acts in liaison with hospital departments to assist in providing professional, technical, administrative and support services.
  19. May represent the facility in development of cooperative relationships with community and in matters involving City, Regional, State and/or Federal agencies.


Qualification Requirements:

  1. A medical degree and Board Certification in Internal Medicine, Family Medicine, Emergency Medicine or equivalent area of specialization and (5) years clinical experience including (3) years supervisory experience.
  2. Unrestricted license to practice medicine in the State of New York, as well as current DEA, BCLS or ACLS certification and Infection Control Certification.
  3. An equivalent combination of training, education and experience in related fields and educational disciplines; and,
  4. Thorough knowledge of the fundamentals of healthcare delivery organization, administration and standards, regulations and laws applicable to hospital operations, knowledge of business and human resources administration principles, management functions, management processes and functions of hospital departments, and ability to direct and supervise personnel.


Departmental Preferences:

  1. Knowledge and experience in patient-centered comprehensive care.
  2. Interest or formal training in correctional health, health inequities, or other relevant knowledge or experience caring for vulnerable populations.
  3. Experience analyzing data from electronic systems.
  4. Experience working with electronic health records to adapt clinical tools and integrate user feedback.
  5. Training in trauma-informed care and/or interdisciplinary care experience.
  6. Collaborative approach to care provision, and drive to advocate and provide care for people impacted by incarceration.
