Pay Range: $26 - $31/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:


  • Assisting clients in receiving services by telephone and in person.
  • Getting clients involved in beneficial activities.
  • Assessing clients and gathering relevant information.
  • Offering information and supporting clients and their families.
  • Contacting and making referrals to other agencies and services.
  • Maintaining accurate records and preparing reports.


Key Requirements and Technology Experience:


  • Key skills; 2 Years of Social Worker experience with Basic computer skills,
  • Education: MSW required.
  • If MSW is not obtained, LCSW will be considered acceptable.
  • Exceptional customer service.
  • Be able to think on their feet.
  • Get along well with the team.
  • Ability to look at a case holistically and Write basic care plans: such that, if exercise is required, will list walk 4x a week for 30 minutes, schedule follow up appointments with specialty doctor every 6 weeks (as reference by doctor in chart), etc.
  • Tools – Epic is a hug plus.
  • Education – MSW Required.
  • Duration – 13 Weeks and possibility of extension and conversion.
  • Hours – 9 AM – 5:30 PM, Monday Friday.
