About the Opportunity

We are seeking a compassionate Internal or Family Practice Physician with expertise in Geriatric Care to join our healthcare team in San Marcos, CA. This role is perfect for a physician dedicated to improving the quality of life for elderly patients through innovative, patient-centered care.

Key Responsibilities

  • Provide primary and acute care for geriatric patients through in-person and telehealth visits.
  • Conduct comprehensive assessments and develop individualized care plans.
  • Collaborate with an Interdisciplinary Team (IDT) to coordinate patient care.
  • Supervise and support Nurse Practitioners and Physician Assistants.
  • Participate in transitions of care, post-hospital visits, and clinical decision-making.
  • Serve as a liaison between outpatient and inpatient specialists.
  • Conduct visits to assisted living and skilled nursing facilities as needed.
  • Review coding, outcomes, and utilization data to improve patient care and efficiency.
  • Participate in night and weekend call rotations (telephonic only).


Qualifications

  • Education: MD or DO degree.
  • Residency: Completion of an ACGME-accredited residency program.
  • Board Certification: Internal Medicine or Family Practice (Geriatrics, Hospice, Palliative Care, Addiction Medicine, or Pain certifications are a plus).
  • Licensure: Active California physician license without restrictions.
  • Certifications: Current Basic Life Support (BLS), State CDS Certification, and DEA Registration.
