Position Summary:
Clinical Hospital Pharmacist by using established policies and procedures is responsible for:
· Drug therapy of assigned patients
· Provision of clinical drug information to meet the needs of the physician, nurse, and patient, education of pharmacy students, residents, and interns
· Distribution of medications.
Duties & Responsibilities:
- Evaluate medication therapy for appropriateness considering patient conditions, safety, efficacy, formulary status, and interactions.
- Provide therapeutic drug level monitoring and communicate with healthcare providers for therapy optimization.
- Identify trends, collect data, and formulate recommendations for practice improvement.
- Present findings to relevant committees and receive exceptional feedback from medical staff.
- Demonstrate accuracy, reliability, and efficiency in dispensing and labeling tasks.
- Complete tasks promptly and meet deadlines consistently.
- Communicate effectively with supervisors and colleagues, suggesting improvements in procedures.
- Provide coaching sessions, contribute to training programs, and engage in professional development.
- Publish or present research findings regularly and participate in multidisciplinary committees.
- Exhibit proficiency in area-specific technology, suggesting enhancements, and solving technical problems effectively.
- Lead or participate in Drug Utilization Evaluation (DUE) or Target Drug Program (TDP) activities.
- Comply with documentation requirements, supervise supportive personnel, and demonstrate behaviors aligned with organizational values.
Experience:
- PGY 2 in Oncology/Hematology or related experience.
Education:
- Pharm.D. degree and residency training or equivalent proven experience is required.
License Certification:
- Possession of or eligible for Florida pharmacist license is required.
