A health system in New Jersey is actively seeking a Licensed Clinical Social Worker (LCSW) to join their Teen Thrive intensive outpatient program (IOP).

Responsibilities:
 The Licensed Clinical Social Worker (LCSW) will:

  • Conduct screenings and intake assessments to evaluate the substance abuse treatment needs of adolescents, including managing admissions and making referrals for additional services
  • Provide ongoing counseling and support to teens, their families, and significant others, addressing psychosocial issues related to substance abuse, based on collaboratively established goals
  • Complete thorough psycho-social assessments promptly to guide treatment planning and interventions
  • Utilize individual, family, and crisis intervention techniques to address problems affecting engagement in treatment and progress, including counseling and discharge planning
  • Serve as a resource to the interdisciplinary team on issues related to substance abuse, coordinating with community resources and referring patients to appropriate services
  • Manage case coordination to ensure adherence to treatment plans and assist families in accessing supportive services and resources
  • Act as a liaison between patients, families, and the healthcare team, facilitating communication and coordinating care across various services and settings
  • Maintain accurate and timely documentation of patient interactions, treatment plans, and progress, fulfilling all reporting and record-keeping requirements

Qualifications:

  • Master's degree in Social Work, Professional Counseling, or a related field from an accredited institution.
  • Current New Jersey license as a Licensed Clinical Social Worker (LCSW) or an equivalent credential
  • 1-2 years experience in a mental health environment, such as a clinic or hospital, providing mental health services to teens, adults, and families
  • Strong verbal and written communication abilities
  • Exceptional interpersonal skills

  • Experience working with teens and adults facing challenges such as homelessness, addiction, emotional difficulties, housing issues, fragmented healthcare, and financial problems
  • Fluency in a second language
