Key Responsibilities:

  1. Patient Care Coordination
  • Serve as the primary point of contact for patients, ensuring seamless communication between providers, staff, and external care teams.
  • Schedule and manage patient appointments, follow-ups, and referrals while optimizing provider schedules.
  • Provide patient education about care plans, medication instructions, and chronic care management programs.
  1. Administrative Support
  • Oversee accurate documentation and compliance with healthcare regulations, ensuring all patient records are up-to-date.
  • Complete and process prior authorizations, FMLA forms, and other administrative paperwork.
  • Manage lab results, imaging orders, and other diagnostic paperwork through the appropriate channels.
  1. Medication Management
  • Handle medication refill requests, verify patient eligibility, and collaborate with pharmacies as needed.
  • Conduct regular checks on sample medications to ensure proper inventory and updates.
  1. Operational Efficiency
  • Utilize healthcare software such as ECW, GAMMIS, or patient portals to manage workflows and improve care delivery.
  • Identify and implement improvements in operational processes, including patient flow and documentation procedures.
  • Assist in developing and maintaining office protocols and standard operating procedures (SOPs).
  1. Team Collaboration
  • Act as a liaison between clinical and administrative staff, promoting clear communication and a cohesive team environment.
  • Participate in team meetings to provide updates on patient care coordination and suggest process enhancements.

Qualifications:

  • Education:
  • High school diploma or equivalent required; Bachelor's degree in healthcare administration, nursing, or related field preferred.
  • Experience:
  • Minimum of 2 years of experience in a healthcare setting, preferably in care coordination or a similar role.
  • Knowledge of insurance verification, prior authorizations, and VFC protocols is a plus.
  • Skills:
  • Proactive problem solver with strong organizational and multitasking abilities.
  • Excellent verbal and written communication skills, with the ability to explain complex processes to patients and team members.
  • Proficient in healthcare technology, including patient portals, scheduling systems, and Excel.
