Key Responsibilities:
- Patient Care Coordination
- Serve as the primary point of contact for patients, ensuring seamless communication between providers, staff, and external care teams.
- Schedule and manage patient appointments, follow-ups, and referrals while optimizing provider schedules.
- Provide patient education about care plans, medication instructions, and chronic care management programs.
- Administrative Support
- Oversee accurate documentation and compliance with healthcare regulations, ensuring all patient records are up-to-date.
- Complete and process prior authorizations, FMLA forms, and other administrative paperwork.
- Manage lab results, imaging orders, and other diagnostic paperwork through the appropriate channels.
- Medication Management
- Handle medication refill requests, verify patient eligibility, and collaborate with pharmacies as needed.
- Conduct regular checks on sample medications to ensure proper inventory and updates.
- Operational Efficiency
- Utilize healthcare software such as ECW, GAMMIS, or patient portals to manage workflows and improve care delivery.
- Identify and implement improvements in operational processes, including patient flow and documentation procedures.
- Assist in developing and maintaining office protocols and standard operating procedures (SOPs).
- Team Collaboration
- Act as a liaison between clinical and administrative staff, promoting clear communication and a cohesive team environment.
- Participate in team meetings to provide updates on patient care coordination and suggest process enhancements.
Qualifications:
- Education:
- High school diploma or equivalent required; Bachelor's degree in healthcare administration, nursing, or related field preferred.
- Experience:
- Minimum of 2 years of experience in a healthcare setting, preferably in care coordination or a similar role.
- Knowledge of insurance verification, prior authorizations, and VFC protocols is a plus.
- Skills:
- Proactive problem solver with strong organizational and multitasking abilities.
- Excellent verbal and written communication skills, with the ability to explain complex processes to patients and team members.
- Proficient in healthcare technology, including patient portals, scheduling systems, and Excel.
