Pride Health is hiring a Licensed Practical Nurse - Home Care to support our client’s medical facility in Newton Center, MA. This is a 13-week assignment (Possibility of extension or conversion) opportunity and a great way to start working with a top-tier healthcare organization


Job Summary:

We are seeking a well-organized Licensed Practical Nurse to join our home care team. In this role, you will provide high-quality nursing care to patients in various community settings, collaborating with an RN case manager to ensure the best possible outcomes for our patients.


Key Responsibilities:

  • Deliver skilled nursing care during home visits.
  • Collaborate with the RN case manager to develop and implement care plans.
  • Utilize electronic documentation to maintain accurate patient records.
  • Communicate effectively with interdisciplinary teams in accordance with agency policies and standards.
  • Manage your schedule and travel to assigned cases as needed.


Qualifications:

  • Current LPN professional license in Massachusetts.
  • Minimum of 1 year of certified home care experience.
  • Ability to travel to various patient locations.

  • Location: Newton Center, MA
  • Job Type: 13 Weeks Contract
  • Shifts: Morning (8:00 AM - 4:30 PM)
  • Pay Range: $1500-$1900 Weekly


*Offered pay rate will be based on education, experience, and healthcare credentials.
