A leading human services organization in New York City is seeking dedicated Case Managers to join their team. These positions are for temp-to-perm roles at two supportive housing sites.

About the Opportunity:

  • Locations: Manhattan, NY
  • Start Date: Immediate
  • Schedule: Monday to Friday, 9:00 am to 5:00 pm
  • Department: Supportive Housing

Responsibilities:

  • Provide case management to assigned clients, including setting and achieving short and long-term treatment goals
  • Coordinate and follow up with medical and psychiatric treatment providers
  • Assist clients in receiving and maintaining entitlements
  • Help clients comply with building rules and government subsidies
  • Maintain accurate case records and documentation
  • Accompany clients to appointments

Desired Skills:

  • Bachelor's Degree
  • Strong understanding of the Housing First model
  • Knowledge of severe and persistent Mental Illness
  • Familiarity with harm reduction and trauma-informed care
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)

Qualifications:

  • Bilingual (English and Spanish)
  • Experience working with individuals diagnosed with severe Mental Illness, Substance Abuse, and/or Co-occurring Disorders
  • Prior experience in Case Management
