Company Description

Community Transitions LLC is a social service company located in the Spokane valley. We specialize in housing for the underserved and low-income populations. We hold 2 contracts. One contract is the Community Choice Guide (CCG) contract. With this contract we work hand in hand with the case managers helping the clients obtain housing, return to independent housing, and become self-efficient. With our CCG clients the task's asked of us can range from independent housing or assisted living searches, Shopping for the client, obtaining an individual provider, helping the client obtain documents, helping the client with their social security application, etc. The other contract we hold is the Governers Opportunity for Supportive Housing (GOSH). With this contract we gain our clients through referrals from Eastern and Western state hospitals. Our goal is to help these individuals become self-efficient in an independent environment with mental health services. With the GOSH contract we work hand in hand with the case managers, State hospital social workers, PACT teams, and insurance companies.

Community Transitions LLC has been stabilizing the underserved and low-income populations since 2018. We work together as a small team, and all take pride in our day-to-day work.

Role Description

This is a full-time on-site role for a Social Worker/housing specialist at Community Transitions LLC in Spokane Valley, WA. The provider will be responsible for providing support and assistance to individuals in need. The role involves assessing clients' needs, coordinating services, and advocating for clients. Jobs may include but are not limited to:

Assisting with obtaining housing, maintaining housing, participating as a mental health team member, developing housing resources in the community, working with clients to develop a housing plan, compiling and assessing eligibility information in compliance with housing regulations, completion of applications for rental subsidies such as section 8 Housing, Shelters, Housing assistance programs, applications for apartments, help Gather required documents such as identification, Social Security card, bank statements, Accompany/transport client to limited appointments (such as move in dates, or tours of apartments as needed), Advocates and negotiates for clients with poor credit and poor housing backgrounds (i.e. evictions or lack of housing tenancy), assists with moving in, ensures client is connected to on-going mental health services (if needed), Conduct in home visits (this will depend on each client).

  • Bachelor's or Master's Degree in Social Work or a related field OR 3+ years in supportive housing, OR 3+ years in caregiving
  • Relevant certifications and licenses as required
