Job Description

  • Interviews patients, and complete intake interview documentation to obtain the most complete case history possible.
  • Effectively utilizes different outreach/support techniques to directly engage clients
  • Serves as a liaison between client, staff, and community members, ensuring client needs are met
  • Provide appropriate resources to assist with clients’ needs
  • Follow up with clients
  • Provides referral services, contacting appropriate social agencies.
  • Prepares narrative, and statistical reports as necessary. May prepare forms of application for services or benefits.
  • Assists in evaluation of client/patient need in order to affect the most beneficial placement

Job Requirements:

  • Knowledge of case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual’s human service needs.
  • Knowledge of mental and emotional disorders and the social, economic, and psychological implications and behavioural impact.
  • Knowledge of methods and techniques of counselling and social casework.
  • Knowledge of family dynamics and reactions to developmental disabilities and mental illness.
  • Knowledge of availability, role and functions of community resources.
  • Knowledge of interviewing techniques.
  • Ability to interpret, explain and implement applicable rules and regulations.
  • Ability to prepare reports.
  • Ability to maintain effective working relationships with clients, families, other service agencies, public and private, as well as state and federal regulatory agencies.
  • Ability to apply interviewing techniques.
  • Possession of a High school diploma.
  • Certified Peer Support or ability to become certified.
