Job Description
The clinical pharmacist at a multispecialty outpatient clinic ensures optimal patient outcomes related to chronic and specialty drug related disease states. Serves as a resource and drug expert to patients, families, providers, nurses and other healthcare professionals. The duties will include but are not limited to
- Works within the health center’s EHR to locate, retrieve or process the appropriate information required for pharmacy operations and compliance.
- Support 340b program operations as needed. This includes but is not limited to identifying patients that would benefit from the services our in-house pharmacy provides as well as educating clinic staff on our in-house pharmacy services.
- Provides direct patient care in an ambulatory setting with a focus on managing chronic and specialty drug related disease states.
- Provides accurate, safe, timely, and appropriate medication therapy based on patient needs.
- Works as an active member of pharmacy clinical programs and collaborates with healthcare providers and in-house pharmacy staff, and places practice priority on the delivery of patient-centered care.
- Reviews and interprets providers’ orders for appropriateness and ensure that drugs are appropriately utilized according to physicians’ instructions. Councils’ patients about their medications and maintains an accurate, up-to-date medication patient profile.
- Performs essential job functions throughout scheduled hours accurately and efficiently, including but not limited to patient interviews and visits, patient mediation adherence and adjustments, and patient medication chart reviews, reconciliations, and documentation.
- Provides drug information such as dosing, compatibilities and appropriate substitutes to other health care practitioners.
- Provides coverage of in-house pharmacy operations and maintains proficiency to practice in several patient care areas and practice assignments.
- Maintains up-to-date knowledge of the profession by complying with National and State Standards of Continuing Education to maintain professional competency and proficiency.
- Participates in clinical programs instituted in the department such as drug-level monitoring, antibiotic monitoring, administering vaccines, floor stock consulting, medication therapy management (MTM) consults and other clinical or compliance services as determined by the Chief of Pharmacy.
- Immediately reports any prescription errors to the Director of Pharmacy. Assists in the completion of all required pharmacy reports.
- Ensures that the quality of professional services is maintained.
- Mentors new staff and assists with training of all staff as requested.
Required Skills/Abilities
- Ability to dispense medications accurately and efficiently
- Active state pharmacy license in good standing
- Ability to read, analyze and interpret prescriptions
- Retail experience and demonstrated leadership ability
- Ability to calculate figures and amounts such as volume. Ability to apply concepts of basic mathematics
- Ability to apply commonsense understanding to carry out instructions furnished in written, oral, or diagram form with mathematical skills
- Ability to work independently and handle problems involving several concrete variables in standardized situations
- Computer skills required: Pharmacy Management Software; Pharmacy Point of Sale Software; Microsoft Office; Electronic Health Record Software.
- BLS/CPR certification
Education and Experience
- License required – South Carolina Pharmacy license required
- PharmD degree required
- One (1) to three (3) years clinical pharmacist experience required
- Completion of PGY-1 Pharmacy Residency, preferred
- Certified through APhA's national immunization program required
Hours
Full-time, Monday – Friday, 8:00 AM – 5:00 PM
Location
Will work a defined schedule weekly at all CareSouth locations based on patient need. Offices located in Bishopville, Hartsville, Society Hill, Chesterfield, Cheraw, Bennettsville, McColl, Latta, Dillon, and Lake View. At the time of posting the immediate needs for scheduling locations is Bishopville, Hartsville, and Latta.
