Who We Are: Siena House is a residential program providing a stable environment of health, support, and education for pregnant women and their babies who are experiencing homelessness or housing instability. Our mission is to give new mothers experiencing homelessness a solid foundation as parents, and their babies the best start in life by providing a supportive environment during pregnancy and a baby’s critical first year of development.


Position Summary: Reporting to the Executive Director (ED), the case manager will act as an advocate for Siena House participants through their proactive and trauma-informed approach to program interventions and services. The case manager will also oversee the Siena House Life Skills and Alumni Programs.


Shift Schedule: Monday-Friday, 9a-5p. Flexibility to adjust as needed for weekend and evening events.


Responsibilities:

Individual Case Management

  • Provide case management services to residential participants and alumni. This includes but is not limited to:
  • Initial enrollment and program exits
  • Ongoing needs assessment
  • Individual service plans
  • Budgeting
  • Information and referrals
  • Crisis de-escalation
  • Safety planning, risk assessment, and mandated reporting
  • Employment and benefits assistance
  • Housing search
  • After-care services
  • Meet with program participants at a frequency determined by their individual needs, no less than weekly.
  • Collaborate with community partners regarding support services for participants.
  • Coordinate case conferencing with outside providers and support systems.
  • Document, via case notes in HMIS, all case management activity within 3 business days and maintain all other case file paperwork and documentation.
  • Engage in the consistent application of evidence-based practices.
  • Proactively and consistently improve the quality and effectiveness of program services.
  • Maintain open lines of communication, and provide regular updates to supervisor.
  • Maintain professional relations and conduct.
  • Transport participants to appointments as needed, using the Siena House vehicle.
  • Other duties assigned.


Life Skills Program

  • Assist ED with developing and overseeing the Siena House life skills curriculum.
  • Identify resources and templates.
  • With the Program Manager, recruit and schedule volunteers to lead classes.
  • Conduct assessments before, during, and after participation in the program.
  • Create an individualized life skills plan for each participant based on assessment results.
  • Teach classes/workshops as needed.


Alumni Program

  • Oversee the Alumni Program, in partnership with the ED, to include regular check-ins, data collection, event planning, and mentor connections.


Desired Qualifications

  • Bachelor’s degree in social work, public health, or a related field. Degree may be substituted with 2 years of experience in a relevant position.
  • Proficient with Outlook, Google Workspace, Microsoft 365, or similar applications.
  • Experience using electronic databases such as HMIS is a plus.
  • Bilingual English-Spanish is a plus.
