Position Overview:

We are seeking a compassionate and dedicated Non-Medical Case Manager to join our Ryan White Program team. The Non-Medical Case Manager will play a crucial role in providing case management and support services to individuals living with HIV/AIDS, helping them navigate the healthcare system, access resources, and achieve optimal health outcomes. This position offers an opportunity to make a meaningful difference in the lives of our clients and contribute to our mission of providing holistic care and support.


Key Responsibilities:

  1. Case Management:
  • Provide comprehensive case management services to clients enrolled in the Ryan White Program, including needs assessment, care planning, and goal setting.
  • Conduct regular assessments of clients' medical, social, and psychosocial needs to develop individualized care plans.
  • Coordinate and facilitate access to medical care, medication adherence support, mental health services, substance abuse treatment, and other support services.
  1. Resource Coordination:
  • Connect clients with community resources and support services to address their housing, transportation, food security, and other basic needs.
  • Assist clients in accessing public benefits, insurance coverage, financial assistance programs, and medication assistance programs.
  • Advocate on behalf of clients to ensure they receive timely and appropriate services and support.
  1. Client Advocacy and Support:
  • Advocate for clients' rights and empower them to actively participate in their care and treatment decisions.
  • Provide emotional support, crisis intervention, and counseling services to clients and their families as needed.
  • Maintain regular communication with clients to monitor their progress, address concerns, and provide ongoing support.
  1. Documentation and Reporting:
  • Maintain accurate and up-to-date client records, including assessments, care plans, progress notes, and service utilization data.
  • Prepare and submit reports on program activities, client outcomes, and compliance with grant requirements.
  • Ensure compliance with confidentiality and HIPAA regulations in the handling of client information.
  1. Collaboration and Networking:
  • Collaborate with interdisciplinary team members, including medical providers, social workers, counselors, and community partners, to coordinate care and support services.
  • Participate in case conferences, team meetings, and trainings to enhance knowledge and skills related to HIV/AIDS care and case management.
  • Build and maintain relationships with community organizations, government agencies, and advocacy groups to enhance service delivery and referral pathways.

Qualifications:

  • Bachelor's degree in Social Work, Psychology, Counseling, Public Health, or related field (Master's degree preferred).
  • Minimum of 3 years of experience providing case management or support services to individuals living with HIV/AIDS or other chronic illnesses.
  • Knowledge of HIV/AIDS care, treatment, and support services, as well as familiarity with the Ryan White Program and related resources.
  • Strong interpersonal, communication, and advocacy skills, with the ability to build rapport with clients from diverse backgrounds.
  • Ability to work collaboratively in a multidisciplinary team environment and effectively coordinate services with external providers and agencies.
  • Proficiency in Microsoft Office applications and electronic health record systems.
  • Bilingual proficiency in English and Spanish preferred.
  • Valid driver's license and reliable transportation required.
