Job Summary:

The Occupational Therapist plays a pivotal role in coordinating and delivering restorative and rehabilitative Occupational Therapy services both at our center and in participants' homes. Working closely with PACE Physicians, rehabilitation staff, and other interdisciplinary team members, the Occupational Therapist aims to optimize participant independence, safety, and performance in activities of daily living (ADLs). They are integral to the efficient operation of rehabilitation department and contribute to fostering collaborative relationships within and outside the organization to uphold Clinic values, team culture, and mission.


Responsibilities:

  • Conduct comprehensive Occupational Therapy assessments to identify participants' needs, preferences, and goals, actively participating in interdisciplinary team meetings to develop individualized care plans.
  • Deliver and document Occupational Therapy interventions in alignment with physician orders and care plans, utilizing appropriate devices, techniques, and activities to enhance muscle re-education, perceptual-motor skills, cognitive function, balance, and endurance.
  • Provide supervision and guidance to Therapy Assistants, including training in implementing Functional Maintenance Programs (FMP).
  • Offer training and instructions to staff, participants, and caregivers/families in Occupational Therapy techniques.
  • Manage equipment procurement, tracking, and maintenance.
  • Maintain open communication with team members, participants, and caregivers, promptly updating the interdisciplinary team on any changes in participants' conditions or functional status.
  • Collaborate with the interdisciplinary team to ensure smooth care transitions between settings.
  • Support enrollment efforts for prospective participants in coordination with the Marketing Team.
  • Participate in end-of-life care coordination and support.
  • Manage direct reports, including training, supervision, and mentoring.
  • Adhere to legal and ethical standards of Occupational Therapy practice.
  • Ensure timely and accurate completion of all required documentation.
  • Uphold privacy and confidentiality standards regarding company procedures and participant information.
  • Adhere to Clinic policies, participate in Quality Improvement activities, staff training, and meetings.
  • Maintain regular communication with supervisors and team members regarding workload and priorities.
  • Complete mandated trainings, education, and occupational health screenings as required.
  • Demonstrate flexibility in performing assignments as business needs evolve.
  • Fulfill other duties as assigned.

Skills:

Required Skills & Experience:

  • Minimum of two years of Occupational Therapy experience in a clinical setting with a frail or elderly population.
  • Strong leadership skills with the ability to motivate and support team members effectively.
  • Proficiency in Occupational Therapy techniques for treating frail, elderly participants with complex needs.
  • Two years of supervisory experience with demonstrated ability in team development.
  • Excellent organizational, communication, and leadership skills.
  • Experience in data-driven decision-making.
  • Ability to work independently with minimal supervision.
  • Proficiency in prioritization within a fast-paced environment.
  • Competency in working with individuals from diverse backgrounds and cultures.
  • Commitment to enhancing the well-being of vulnerable seniors.

Education:

Required Education:

  • Bachelor’s Degree in Occupational Therapy from an accredited program meeting AOTA and ACOTE standards.

Required Certifications & Licensure:

  • Initial registration with the National Board for Certification in Occupational Therapy.
  • Licensure or eligibility for licensure as required in the State of California.
  • Valid State Driver’s License with a clean DMV record and recent Vehicle Insurance.
