HCP Talent is collaborating with a Long-Term Care/Rehabilitation (720 beds) in the Bronx, NY 10469 that is seeking a Social Worker (SW) for a full-time Day shift (7a-3p) M-F position.

SW Duties and Responsibilities:

  • Responsible for 1 sub-acute unit and 1 long term care unit (80 beds in total)
  • Ensure that all Social Work documentation is completed in accordance with facility policy and procedure as applicable
  • Coordination of Interdisciplinary Discharge Planning (Knowledge of community resources
  • Demonstrate compliance with government and accrediting agency standards and regulations pertaining to Social Work
  • Interface with facility departments and outside agencies in order to provide a complete plan of care and to facilitate services.
  • Liaison between family and other interdisciplinary team members
