Role Overview:

As a Home Infusion Pharmacist, you will play a vital role in the delivery of exceptional patient care. Your responsibilities will include:

  • Dispensing and compounding medications for home infusion therapy.
  • Ensuring the safe and effective administration of medications to patients.
  • Providing pharmaceutical care and counseling to patients and their caregivers.
  • Monitoring patient progress and making necessary adjustments to treatment plans.
  • Collaborating with the sales team, physicians, nurses, and other healthcare professionals to ensure optimal patient outcomes.
  • Maintaining accurate patient records and complying with all relevant regulations and standards.
  • Collaborating with cross-functional teams, including sales, intake, operations, and nursing, to ensure the seamless delivery of services.

Responsibilities:

Patient Care:

  • The Home Infusion Pharmacist will work as part of an interdisciplinary team to ensure the delivery of appropriate, safe & timely compounded medications, non-compounded medications & infusion supplies
  • Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug Administration, the Drug Enforcement Administration, and the United States Pharmacopeia
  • Maintains certification and credentials in accordance with the Board of Registration of Pharmacy and adheres to policies and procedures
  • Reviews and verifies medication orders
  • Assist patients with navigating insurance coverage and obtaining necessary authorizations.
  • Advocate for patients with insurance companies and financial assistance programs to ensure access to medications.
  • Provides education on medication & their administration to patients, their caregivers as well as nursing staff
  • Collaborates as part of an interdisciplinary team to advocate for patient outcomes & adherence to therapies
  • Reviews / interprets orders to make decisions on medication changes or recommendations on medication changes
  • Documents and communicates pertinent patient information and discussions with the healthcare team and pharmacy staff
  • Compounds, checks and verifies sterile compounded products
  • Prepares, checks and verifies non-sterile products, pumps and supplies
  • Supervises the activities of students, technicians, and residents within the pharmacist area of practice
  • Participates in the onboarding and training of new hires and precepts pharmacy students, technicians, and/or residents through core clinical rotations or learning experiences
  • Participates in department initiatives including: Performance Improvement, Policy & Procedure review, Workflow Assessment & Enhancement, and Financial Performance
  • May make recommendations for solving problems of moderate complexity and importance
  • May set priorities and organize work within general guidelines
  • Seeks assistance when confronted with difficult and/or unpredictable situations
  • Work progress is monitored by supervisor/manager
  • Written Communications: Ability to communicate clearly and effectively in written English with internal and external customers
  • Oral Communications: Ability to comprehend and converse in English to communicate effectively with staff, patients, families and external customers


Navigating Complex Prior Authorization Processes:

  • Insurance companies have increasingly complex and ever-changing prior authorization requirements.
  • Pharmacists must constantly adapt their strategies to navigate these complexities, which often involve:
  • Obtaining and submitting extensive medical documentation
  • Communicating effectively with sales, physicians, nurses, and insurance representatives
  • Identifying and appealing denials
  • Staying updated on the latest payer policies and guidelines


Addressing Treatment Emergencies:

  • In urgent situations, pharmacists may need to quickly obtain prior authorizations for critical medications.
  • This requires innovative solutions to expedite the process, such as:
  • Utilizing expedited review pathways
  • Escalating cases to appropriate levels within the insurance company
  • Exploring alternative treatment options if necessary


Improving Patient Access to Care:

  • Identify delays in obtaining prior authorizations.
  • Implement innovative initiatives to streamline the process to improve patient access to timely and effective infusion therapy.


Pharmacy Operations:

  • Manage inventory and procurement of infusion medications and supplies, ensuring proper storage and handling.
  • Participate in quality assurance programs and continuous improvement initiatives.


Professional Development:

  • Stay current with advancements in infusion therapy pharmaceuticals and techniques.


Qualifications:

  • BS in Pharmacy or PharmD degree
  • Active and in good standing pharmacy license
  • 3 years Sterile Compounding Experience
  • Ability to develop strong relationships with diverse groups of professionals
  • Experience with computer systems required, including web-based applications and some Microsoft Office applications which may include Outlook, Word, Excel, PowerPoint or Access
  • Decision Making: Ability to make decisions that are guided by general instructions and practices requiring some interpretation
  • Problem Solving: Ability to address problems that are varied, requiring analysis or interpretation of the situation using direct observation, knowledge and skills based on general precedents
  • Independence of Action: Ability to follow precedents and procedures
  • Knowledge: Ability to demonstrate full working knowledge of standard concepts, practices, procedures and policies with the ability to use them in varied situations
  • Team Work: Ability to work collaboratively in small teams to improve the operations of immediate work group by offering ideas, identifying issues, and respecting team members
  • Customer Service: Ability to provide a high level of customer service to patients, visitors, staff and external customers in a professional, service-oriented, respectful manner using skills in active listening and problem solving
  • Ability to remain calm in stressful situations
  • Adaptability: Ability to work in a fast-paced environment and adapt to acute change
  • Work requires close attention to task for work to be accurately completed
  • Medium work: Exerting up to 50 pounds of force occasionally and or up to 20 pounds of force frequently
  • There may be occasional bending neck, bending waist, Fine Manipulation using both hands, Pushing/Pulling using both hands
