Job Summary: Savie Health seeks a dedicated and culturally competent Bilingual Bicultural Case Manager to spearhead our new Enhanced Care Management (ECM) program. This pivotal role focuses on providing ECM to community members in Lompoc and throughout Northern Santa Barbara County. The ideal candidate will manage a caseload of approximately 30 members currently eligible for but not receiving ECM services, effectively launching and leading this vital new initiative.

Key Responsibilities:

  • Initiate and establish the ECM program, developing protocols and best practices for effective management and care coordination.
  • Conduct thorough assessments to identify individual needs and eligibility for ECM services.
  • Develop, implement, and monitor personalized care plans for each member in the caseload.
  • Maintain regular contact with members, aiming to engage at least 60 members during the grant period, with comprehensive assessments and care plans completed for 30 members.
  • Work closely with healthcare providers, social services, and other agencies to coordinate services and advocate for member needs.
  • Document all member interactions, services provided, and outcomes achieved in the healthcare system.
  • Evaluate the effectiveness of care plans regularly and make adjustments as necessary.
  • Achieve target outcomes, including significantly reducing emergency room visits among members receiving ECM.
  • Participate in ongoing training and professional development related to ECM and care coordination.

Qualifications:

  • Proficiency in both English and Spanish is required.
  • Deep understanding of cultural nuances within the local community.
  • Bachelor’s degree in Social Work, Psychology, Nursing, or a related field.
  • At least two years of experience in case management, preferably within a healthcare or social services setting.
  • Strong organizational and time management skills with the ability to manage a substantial caseload effectively.
  • Excellent interpersonal and communication skills, with the ability to work effectively with diverse populations.
  • Knowledge of local healthcare systems, social services, and additional resources beneficial to members.
  • Proficiency with electronic health records and case management software.
