This job is predominantly in the field servicing their San Gabriel/Pomona Regional Center that includes Pomona, El Monte and Foothill regions.

Job Duties & Responsibilities:

  • Responsible for the entire case management process, including assessment and evaluation, planning, authorizing, advocacy, monitoring, reviewing documentation for completion.
  • Responsible for providing a safe and welcoming environment for individuals , developing case plans according to the needs of each individual and providing ongoing support as they work toward achieving goals.
  • Conduct initial and ongoing assessments and follow-up visits to each individual.
  • Assess individuals’ needs and link them with outside services such as primary medical care, outpatient treatment, government financial assistance etc.
  • Maintain all documentation and contacts as required by law, regulation and policy
  • Advocate on behalf of the organization’s individuals to protect their rights under the law and ensure that they receive those services and benefits to which they are entitled.
  • Collaborate with other departments and outside providers to support and advocate for the organization’s individuals.
  • Respond to crises, including providing de-escalation intervention and support to highly agitated individuals.
  • Keep supervisor informed of the status of the caseload and unmet needs in the community.
  • Ability to cover evening and weekend shifts with certain caseloads.


Job Qualifications:

  • Bachelor’s degree in Counseling, Psychology, Social Work, Sociology from an accredited college or university.
  • Master’s degree (strongly preferred) in Counseling, Psychology, Social Work, Sociology from an accredited college or university.
  • 3+ years customer/patient service experience, preferably in human/social services.
  • Knowledge of community resources and referral networks related to mental/behavioral health services.
  • Familiarity with relevant legal and regulatory frameworks governing behavioral health practice.
  • Experience in a non-profit, case management, social services, and mental/behavioral health.
  • Proficient in crisis intervention techniques and risk assessment.
  • Excellent communication and interpersonal skills for effective client engagement and collaboration with interdisciplinary teams.
  • Ability to maintain professional boundaries, handle sensitive information, and adhere to ethical guidelines.
  • Demonstrated commitment to ongoing professional development and continuous learning.
