Job Overview

As a Physical Therapist at Klamath Health Partnership, Inc, a Federally Qualified Health Center (FQHC), you will play a vital role in providing high-quality, compassionate care to a diverse patient population. You will work within an interdisciplinary team to assess, develop, and implement personalized physical therapy plans aimed at improving patients’ mobility, pain management, and overall quality of life. This role is ideal for someone who is passionate about community health and dedicated to serving underserved populations in a supportive, collaborative environment.


Key Responsibilities

  • Conduct comprehensive physical therapy assessments, including reviewing medical history, physical exams, and functional testing.
  • Develop individualized treatment plans tailored to each patient’s specific needs and goals, using evidence-based practices.
  • Deliver therapeutic interventions such as manual therapy, therapeutic exercises, and patient education to restore function and alleviate pain.
  • Educate patients and families on managing symptoms, preventing further injuries, and promoting a healthy lifestyle.
  • Collaborate closely with a multidisciplinary team, including primary care providers, behavioral health specialists, and case managers, to coordinate comprehensive patient care.
  • Monitor and document patient progress, adjusting treatment plans as necessary to ensure optimal outcomes.
  • Maintain accurate, thorough, and timely documentation in compliance with FQHC standards, HIPAA, and other regulatory requirements.
  • Participate in quality improvement initiatives, community health programs, and staff training as needed to support clinic goals.


Qualifications

  • Education: Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
  • Licensure: Current state licensure as a Physical Therapist.
  • Experience: Experience in a community health setting or FQHC is preferred but not required.
  • Skills: Strong interpersonal and communication skills, proficiency in a variety of therapeutic techniques, and an ability to work effectively within a team.
  • Other: Bilingual or multilingual skills are a plus, reflecting the diverse population served.
