- Evaluate and assess patients' physical and functional abilities.

- Develop individualized treatment plans to address patients' specific needs and goals.

- Implement therapeutic interventions to enhance patients' daily living skills, motor functions, and cognitive abilities.

- Educate patients and their families on techniques and strategies to improve their daily living and quality of life.

- Monitor and document patient progress, adjusting treatment plans as necessary.

- Collaborate with other healthcare professionals to provide comprehensive care.

- Maintain accurate and timely records in compliance with healthcare regulations.

- Participate in team meetings and case conferences as needed.


Qualifications:

- Master's or Doctoral degree in Occupational Therapy from an accredited program.

- Valid state licensure as an Occupational Therapist.

- Previous experience in home health care is a plus.

- Excellent communication and interpersonal skills.

- Ability to work independently and as part of a multidisciplinary team.
