Accountability: Accountable to Housing Program Manager, with supervisory assistance from Housing Specialist Lead

Duties/Responsibilities:

  • Meeting with clients regularly to offer one-on-one support and to create a case plan that addresses their housing barriers and fits the needs of the neighbor.
  • Meet with new Menlo Neighbors within 1 week of entry to complete a case management assessment.
  • Reporting any unusual behavior, conflicts to relevant team members. Conducting assessments and providing feedback to the client, and other staff. Coordinate and communicate with other agencies and organizations throughout the area as needed when it comes to the needs of the neighbor.
  • Researching affordable housing leads
  • Maintains composure in highly stressful or adverse situations.
  • Conducts self within appropriate and expected professional boundaries and policies.
  • Maintains confidentiality while assessing, planning, implementing, monitoring and evaluating actions required to meet the client's housing needs.
  • Ensure prompt case notes, documentation, and data quality within HMIS (Homeless Management Information Systems), organizational data, and case management systems.
  • Represent Cultivate Initiatives to partners, Joint Office of Homeless Services, and the broader community.
  • Attend all assigned training and meetings, unless otherwise approved.
  • May participate in special projects and activities. May perform other duties as requested by the supervisor.

Required Skills/Abilities:

  • Active listening skills/commitment to fully understanding the client’s point of view.
  • Ability to communicate difficult and complex issues to clients who possess a range of listening and processing challenges.
  • Ability to maintain a non-judgment, non-reactionary, and non-defensive communication style during challenging discussions.
  • Exceptional personal skills in working with a diverse community and establish and maintain effective working relationships with other employees and residents.
  • Ability to apply critical thinking skills to make sound judgments in developing and time-sensitive situations.
  • Ability to responsibly prioritize competing tasks and follow through in a timely manner.
  • Capacity to follow meticulous record-keeping/reporting protocols.

Preferred Skills:

  • At least 1 year of Shelter work or some Case Management Experience required
  • Previous experience working with HMIS and/or Apricot.
  • At least two years of relevant experience in providing services to low-income individuals.
  • Knowledge of community resources, social services, and benefits administered by the county.
  • High School diploma preferred

Requirements:

  • Being physically present at the Menlo Park Village for all scheduled shifts.
  • Ability to maintain frequent use and awareness of work communication ranging from Slack, email, and google voice software.
  • We ask all staff be available for our morning briefing and afternoon wind downs, at 8:30 and 4:15 everyday.

Job location:

In-person in the office in East Portland.

Compensation/Benefits:

  • Full-time, non-exempt position at a rate of $24-26.00 hourly DOE
  • 100% employer-paid premiums for medical, vision, and dental insurance.
  • Eleven paid holidays, vacation, and sick time.
