Job Duties & Responsibilities:
- Provide Individualized Diabetes Education: Deliver tailored education to patients based on their specific needs and assessments.
- Create Nutrition Care Plans: Develop and implement comprehensive nutrition plans considering medical, diet, and social histories.
- Integrate Clinical Goals: Combine pathophysiology and patient-specific clinical goals into effective nutrition recommendations.
- Teach and Instruct: Use the teach-back method to ensure patient understanding of diabetes management and glucose monitoring.
- Support Goal Setting: Assist patients in setting and achieving personal nutrition and exercise goals, with regular follow-up for success.
- Identify and Address Barriers: Recognize and resolve barriers to patient care goals, such as financial or knowledge deficits.
- Maintain Accurate Records: Chart education and updates in Electronic Medical Records (EMR) and communicate patient progress to healthcare teams.
- Follow Up with High-Risk Patients: Provide continuous support and ensure follow-up appointments for high-risk patients, reporting issues to physicians.
- Educate Clinic Staff: Present educational information to staff and update nutrition education materials as needed.
- Collaborate for Patient Care: Work with healthcare teams to plan and evaluate patient care, ensuring effective communication and interdisciplinary cooperation.

- Monday-Friday, 8am-5pm

- Methodist Physicians Clinic, Indian Hills- 8901 W Dodge Rd, Omaha, NE
Salary:
- $24.20-$38.70, will be based on work history/experience
Job Qualifications:
- Educational Background: Bachelor’s Degree required, preferably in Nutrition and Dietetics.
- Clinical Experience: 3 years of previous clinical experience preferred.
- Certification: Certified Diabetes Care and Education Specialist (CDCES) through the Association of Diabetes Care & Education Specialists (ADCES) preferred.
- Professional Credentials: Registered Dietitian (RD), Registered Dietitian Nutritionist (RDN), or Registered Nurse (RN) preferred.
