Case Manager


San Diego Youth Services seeks a Case Manager in the Achievement Center Program to be based at the Spring Valley office location. This is a full-time & non-exempt position reporting to the Achievement Center Program Manager.

DEPARTMENT DESCRIPTION OR PROGRAM DESCRIPTION HIRING FOR

This after-school rehabilitative program provided youth services and empowerment utilizing positive youth development, restorative practices, and trauma-informed care to support youth to successfully complete terms of probation to prevent further justice involvement.


CONTEXT OF THE POSITION

Accountable to the Achievement Center Program Managers to provide comprehensive trauma-informed services. The goal of the Achievement Center is to provide supportive services to East County youth 19 and under currently in the Juvenile Justice System or at risk of being involved in the Juvenile Justice System. This includes youth who have committed an offense and/or are currently on Juvenile Probation and working on probation requirements. The Achievement Center provides services in the center that include case management, screening possible intakes, providing intake assessments, crisis management, creating a service plan in partnership with the youth and their parent/legal guardian, facilitating appropriate referrals, as well as community outreach. In addition, responsible for the facilitation and coordination of group services including alcohol and other drug use, teen pregnancy prevention, self-esteem, positive choices, family support, ILS, delinquency prevention, gender-specific groups, and anger management.


Essential functions include:

  • Develop, implement, and evaluate an integrated approach for youth, some with co-occurring disorders, in quality trauma-informed youth programs, which adhere to and uphold the rigorous standards of San Diego Youth Services as mandated by its mission, purpose, guiding principles and standards of quality.
  • Attend and actively participate in all required agency and contract training and meetings.
  • Maintain clear, timely, effective, and open professional communication within the team and among community partners, especially Probation, and referral sources. Includes providing timely progress reports to referral sources.
  • Serve as a role model for youth in the juvenile justice system and their families.
  • Provide initial screening, comprehensive family-based assessment, short-term crisis intervention services, and referrals. Collect input from as many of the following as possible: school, health, law enforcement, and faith community.
  • Develop and maintain helping relationships with parents or guardians emphasizing client accountability. Work with youth and family to continue to re-assess and refine the service plan approach throughout the youth’s enrollment in the program.
  • Maintain data collection and contract compliance of records within contract timelines as required. Maintain accurate and timely uniform case file documentation.
  • Provide individual/family case management services that are strengths-based and family-centered to assure youth’s successful program completion.
  • Maintain current knowledge of resources and treatment providers within the community to support youth and family needs and link families with any necessary supportive or wrap-around services, following up on linkage.
  • Coordinate meeting transportation needs of youth in the program.
  • Provide resources and facilitated referrals for positive youth development activities including leadership activities and/or youth job development and training.
  • Create and lead recreational activities.
  • Assist the Youth Support Partners in the supervision of the youth while in the center and/or on outings.
  • Facilitate trauma-informed and strength-based groups on a variety of topics.
  • With other staff, participate in the maintenance of the day-to-day cleanliness, safety, and order of the physical facility, grounds, and vehicle. Maintain a warm and welcoming environment.
  • Maintain confidentiality and privacy standards of service-partner’s health-protected information and other applicable information and material in accordance with agency policies, and applicable regulations and laws including HIPAA.
  • Welcome and support volunteers in the organization and assist them in accomplishing their services whenever applicable.
  • Provide and Coordinate transportation needs of youth in the program.

PREFERRED PROFESSIONAL EXPERIENCE AND QUALIFICATIONS

  • Must have at least three years’ experience working directly with juvenile justice-involved youth and at-risk youth and families, preferred lived experience as a consumer or caregiver of a consumer of children’s mental health services, the juvenile justice system, or Special Education.
  • Preferred: Bachelor's Degree in Psychology, Sociology, Social Work, or closely related social-service field; knowledge of East County resources; ability to de-escalate and respond to crises appropriately.
  • Develop or possess working knowledge of trauma-informed care and work with a multidisciplinary team in a diverse setting that serves people of all backgrounds, cultures, and sexual orientations. Strong professional oral and written communication skills.
  • Ability to work with minimum supervision and frequent interruptions, while maintaining a professional demeanor.
  • Must be computer literate. Availability to work evenings Monday through Friday. Must maintain CPR/First-Aid Certificate, have valid CA driver’s license and reliable vehicle and relevant auto insurance, plus a clean driving record (mileage reimbursed).
  • Must successfully complete & maintain background check, drug test, and TB test.
  • Must maintain CPR and First Aid Certification.

Salary Range: $20.00 – 23.00/hour
