Job Description:

Join our team as a Physical Therapist working in Home Health, where you'll provide excellent Physical Therapy services to patients in the comfort of their homes.

Your role involves assessing patients' functional capacity, creating treatment plans to enhance their physical and cognitive abilities, and ultimately improving their quality of life.


Required Qualifications:

To be eligible, you need a Master's degree or higher, a valid Physical Therapist license in Pennsylvania (PA), and a Regular Driver's License. Additionally, you should have over a year of experience in a Healthcare Setting.


Preferred Qualifications:

Candidates offering their PT license number will be prioritized.


Key Responsibilities:
  • Conduct Physical Therapy assessments and develop customized treatment plans.
  • Maintain patient records and monitor changes in conditions.
  • Provide patient and caregiver education.
  • Supervise therapy and nursing assistants.
  • Prepare detailed reports and contribute to team development.

Schedule and Shift Details:

Work Monday to Friday, 8:00a/9:00a-5:00p, one weekend per month (Saturday & Sunday day shifts) with scheduled patients, two holidays. No on-call duty.


Travel:

Local travel to patients' homes required.
