Summary:
Under the direction of the Supervisor and/or Program Manager, the Case Manager is responsible for screening and eligibility, service coordination, and triage for individuals, families, and youth seeking services through the Coordinated Entry System.

Essential Functions:
  • Coordinate with project staff and partners to provide housing and supportive services.
  • Update participant's housing status within twenty-four hours of any change.
  • Ensure information is uploaded into HMIS upon referral reception.
  • Assist with Shallow Subsidy referrals process.
  • Create Status Assessment Update for significant client changes within 24 hours.
  • Assist clients in applying for units and attending landlord interviews.
  • Prepare and update housing plans and budgets for clients.
  • Attend weekly case conference meetings.
  • Verify program eligibility documents.
  • Maintain accurate client documentation for service coordination.
  • Enter data into HMIS promptly.
  • Provide necessary training and support to staff and project partners.
  • Prepare reports as per funding requirements.
  • Represent the Agency professionally at meetings and events.
  • Build trusting relationships with clients and stakeholders.
  • Perform home visits and case conferencing as required.
  • Other duties as needed.

Minimum Qualifications:
Associate degree in social services or related field, with 1-2 years of relevant experience. Working knowledge of Microsoft programs, case management, and documentation. Ability to work with diverse populations and communicate effectively. Background clearance and relevant certifications required.
  • Bilingual proficiency.
  • Ability to adapt to a fast-paced environment.
  • Strong time management and critical thinking skills.

Work Environment:
Client-friendly settings, occasional travel required, may work in noisy environments.

Physical Requirements:
Sitting, standing, typing, walking, and carrying up to 25 lbs.

Mental Requirements:
Handle distractions, interruptions, and prioritize effectively.
