POSITION: Social Worker at Stony Brook Southampton Hospital

The Social Worker is a key member of the care team, responsible for planning and delivering social work services tailored to the needs of patients and their families. The role involves working closely with the team to ensure patient health goals are met effectively.

RESPONSIBILITIES:

  • Conduct psychosocial assessments for cases meeting High-Risk Criteria.
  • Identify and address discharge barriers for patients and families.
  • Collaborate with the team on discharge plans for complex psychosocial issues.
  • Offer information and referrals based on community resources and patient insurance coverage.
  • Act as a link between community agencies and the hospital.
  • Supervise social work students when required.
  • Perform any other assigned duties.

REQUIRED SKILLS/EDUCATION:

  • Must be NY State Licensed in Social Work (LMSW or LCSW).
  • Master's degree in social work.
  • Experience in patient discharge planning.
  • Knowledge of healthcare legal aspects.
  • Strong communication skills.

PRE-EMPLOYMENT REQUIREMENTS:

  • Successful completion of a pre-employment physical examination and medical clearance.
  • Passing a 4-panel drug screen.
  • Meeting Regulatory Requirements for pre-employment checks.
  • Valid New York State license(s)/certificate(s) must be provided.
